Pay range: Low $65,586 per year High $72,145 per year
A hiring bonus of $5,000 is available to external candidates with $2,500 paid upon hire and $2,500 paid upon completing 6 months of employment with St. Francis House.
There is a tuition reimbursement program of $25,000 that will be paid in increments of $5,000 at the end of each year of employment for the first five years of employment.
Schedule: Full time, 40-hour workweek scheduled between the hours of 7:00 AM and 5:00 PM, Monday through Friday. Work hours may vary depending on the operational needs of the program, department, or agency. On-call rotation is also required, which may include evening and weekend hours.
Job Summary: The Clinical Case Manager works as part of the Residential Services team to provide direct support to formerly homeless individuals who have transitioned into permanent housing. They operate from a low-barrier, housing first approach, to ensure residents develop skills and have access to resources they need to live increase their stabilization and maintain permanent housing The Clinical Case Manager works as part of a stabilization team to provide wrap around case management services, crisis intervention, triage and clinical consultation to stabilize residents who are currently or previously used substances, have severe mental illness, and/or have complex medical conditions. The Clinical Case Manager works collaboratively with other departments at St. Francis House to help stabilize residents in behavioral health, income, and housing.

Essential Duties / Responsibilities
- Welcomes new residents into housing and provide services and support that foster a smooth transition from homelessness to a stable living environment in permanent supportive housing.
- Conducts assessments with each resident to determine their needs through engagement and strength-based process and creates individualized service plans.
- Carries a caseload of approximately 10 complex individuals who have high levels of service needs at any given time.
- Collaborates with stabilization Case Managers to provide wrap-around services and support around mental illness and substance abuse.
- Provides individual as well as group intervention around co-occurring disorders.
- Connect residents to treatment and service resources.
- Utilizes Best Practice methods of clinical practice including motivational interviewing and harm reduction.
- Maintains real-time, accurate records of residents’ assessments, case notes, and service plans in client database.
- Works with AD to provide group consultation to case managers around substance abuse and mental health of the residents
- Coordinates with external agencies to case conference around residents with nontraditional needs. Incorporates low threshold case management support within counseling services.
- Engages in emergency mental health actions (Section 12, BEST team calls) as needed.
- Actively participates in individual supervision, team meetings, case conferences and presentations.
- Works collaboratively with SFH Workforce Development, Recovery Support Center, and other on-site services and community to develop, promote, and facilitate positive activities for residents living in St. Francis House housing communities.
- Adheres to the agency code of conduct.
- Performs other duties as assigned.
Required Skills & Abilities:
- Knowledge of trauma-informed, recovery-focused, person-centered models of care.
- Knowledge of de-escalation techniques or crisis intervention.
- Knowledge of mental health disorders as defined by the DSM-V.
- Working Knowledge of Best Practice approaches in working with SMI and SUD.
- Highly reliable, proactive, independent, consistent, and responsible.

- Working knowledge of Social Security disability programs and other public benefits’ eligibility criteria, application processes and documentation requirements.
- Master's degree in social work, Counseling, or related field.
- LCSW required or eligible within 3 months of hire; LICSW preferred.
- Minimum of two years providing direct service with vulnerable populations. Internship experience is considered.
- Experience working directly with people experiencing substance use disorder and homelessness or who have recently exited homelessness, preferred.
- Lived experience of homelessness and/or recovery a plus.
- Bilingual (Spanish), preferred.
