FT: 6a-6p, Rotating Shift, every other weekend

Job Duties
The LPN is responsible for coordinating with the health care team to deliver quality care and restorative services to maximize resident health potential, functional capacity, and quality of life. Provides direct and indirect nursing care, resident assessment, care planning, intervention and evaluation to meet resident needs. The LPN is responsible for the direction and supervision of nursing assistants and ensuring the provision of quality care and health services and compliance with professional nursing standards of practice and state and federal regulations. Ability to represent the corporation in a professional manner and maintain a professional appearance; leadership ability and willingness to work harmoniously with staff and managers; good human relation skills; strong verbal and written communication skills; strong organizational and documentation skills; ability to maintain confidentiality; highly motivated, energetic and compassionate. Reliable work ethic; good attendance and punctuality.
Qualifications
- Graduated from an accredited LPN Program.
- Holds a current unencumbered license as a Licensed Practical Nurse.
- Has one year or more of experience as an LPN.
- Long-term care and advanced stage dementia experience a plus
- Current Health Care Provider CPR Certification
Physical Requirements: Ability to stand and walk for extended periods; ability to lift and assist with resident transfers.
