What are the responsibilities and job description for the Licensed Master Social Worker (LMSW) position at St. James Rehabilitation and Healthcare Center?
St. James Rehabilitation & Healthcare Center is seeking a full-time Social Worker for our skilled nursing facility. The ideal candidate will have some experience in long-term care/ sub-acute setting and must have previous healthcare experience.
Duties:
- Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotion needs of the resident
Requirements:
Bachelor's Degree and / or Master's Degree in Social Work
