What are we looking for?
We are looking to add a Program Coordinator to our team. As the Program Coordinator you will be expected to monitor and evaluate all aspects of assigned program operations to ensure quality of service for all involved participants; ensures that facilities are maintained safely and cleanly and in accordance with the applicable regulations. Supervises and coordinates the day-to-day operation of the group home/residential setting and performs other job related duties as assigned
Your overall essential responsibilities will be the following:
- Supervise the day-to-day operation of multiple group homes and/or residential settings assigned.
- Supervises the Residential Team Leaders.
- Serve as liaison with community providers, case managers, group homes, parents and inter-departmental services to facilitate the coordination of services to the participants as requested, as well as increase and improve participant involvement.
- Ensure that the homes are meeting the state, federal, and local requirements.
- Acts as the Lead Clinical Coordinator of the ISP team.
- Coordinate and write the Individual Service Plans and ensure implementation
- Oversee the financial needs of the consumers in the homes.
- Conducts monthly and regular visits to the day program.
- Ensure that all medical appointments and treatments are maintained.
- Prepare and review relevant documents and reports.
- Coordinate in-house staff training events and conduct training on the ISP implementation and documentation.
- Train and evaluate staff.
- Ensure staff compliance with all acceptable program practices and procedures.
- Ensures quality care in regard to the personal needs; clothing, hygiene, food, supplies, and home environment for the individuals served.
- Troubleshoot and/or resolve any issues or areas of concern that arise when dealing with the community.
- Attend and participate in all relevant agency events, workshops, training and meetings.
- Provide coverage when there is a staffing need.
- Provide 24/7 on-call coverage for assigned locations.

All SJCS employees must:
- Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
- Meet SJCS and the state funding authority requirements of criminal background and record checks.
Physical and environmental conditions include:
- May lift a minimum of 35 pounds, to position and/or transfer persons with disabilities.
- Able to sit, stand, bend, stoop, reach and walk for long periods
- May be required to travel up to 50-65%. With possible overnights
- Work environment involves risks or discomforts, which require special safety precautions e.g. safely operating a motor vehicle, exposure to aggressive human behavior. Long and extended hours and travel required.
The hiring rate for this position ranges is $65,000.

Qualifications
You will catch our eye if you have the following knowledge, skills, and abilities:
- Bachelor’s degree as regulations requires per state;
- One or more years’ experience related experience working with people with disabilities
- Valid health certificate that meets the requirements for the jurisdiction(s) in which the employee works.
- Experience with on-site programs preferred.
- Valid local driver’s license with good driving record.
- Reliable means of transportation.
- Or equivalent technical training, education, and/or experience.
Extra credit will be given if you have the following experience, education, and requirements:
- Ability to gain thorough knowledge of St. John’s Community Services policies and procedures.
- Ability to act as a representative of St. John’s Community Services to the public.
- Thorough knowledge of program practices and procedures.
- Ability to effectively communicate with individuals from diverse backgrounds.
- Ability to effectively supervise, train, evaluate and communicate with program staff.
- Ability to research, prepare, review and maintain reports and documents.
- Ability to work independently with minimum supervision.
- Ability to use sound judgment when making decisions.
- Ability to pass a criminal background check; and
- Skilled in the operation of relevant computer systems, including hardware and software, and office machines.
- Ability to maintain effective working relationships with associates, subordinates, representatives from other agencies, and the general public
