CASE MANAGER - INDEPENDENT LIVING SERVICES DEPARTMENT
St Johns County Council on Aging (COA)
20 hrs/wk with room for increased hours as caseload permits - Monday – Friday
Essential Duties and Responsibilities:
- Perform in-home client assessment and reassessment, care plan development, care plan review, and services management for clients 60 years of age and older and their caregivers.
- Maintain case files and compliance with all COA policies/procedures and Department Of Elder Affairs (DOEA) requirements.
- Respond to clients’ calls, follow-up on inquiries and services, and provide community referrals including Adult Protective Services (APS).
- Attend trainings, staff meetings and conferences.
- Perform related duties as assigned by supervisor.
Qualifications and Work Experience Requirements:
- Excellent verbal and written communication skills; ability to effectively communicate with clients.
- Excellent computer skills in MS Office (Word and Excel), and G Suite Tools (Gmail, Drive, Sheets, and Calendar). Ability to learn computer software programs related to client assessments and services.
- Must be able to work under pressure and meet deadlines while maintaining a positive attitude.
- Ability to conduct home visits, develop care plan, schedule follow-up services and be knowledgeable of community resources.
- Bachelor’s degree in social work, psychology, nursing, or related field required or relative experience may be substituted.
Physical Requirements:
- Ability to safely and successfully perform the essential job functions consistent with meeting qualitative and/or measurable productivity standards.
- Ability to maintain regular, punctual attendance consistent with the COA policy.
- Must be able to lift and carry up to 20 lbs.
- Must be able to operate machinery and equipment such as but not limited to: computer, copier, fax.
- Ability to sit for long periods of time.
- Must be able to listen and speak clearly.
- Valid Driver’s License.
