Description
Provides inpatient services, including assessing, diagnosing, prescribing, treating and educating patients and families. Works collaboratively with physicians, physician assistants and other clinicians in a team approach to patient care.
ESSENTIAL FUNCTIONS
Patient Assessment & Admissions to Hospitalist Service
Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation and cultural factors; and performing appropriate physical examination and plan of care.
· Orders/performs appropriate laboratory and radiology diagnostics as well as other screening tests. Interprets these tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
Manages Existing Patients in Collaboration with Hospitalist
· Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Performs appropriate medical procedures within scope of practice. Delegates direct and indirect patient care activities to appropriate personnel. Provides instruction/education to patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention.
· Communicates in a timely manner follow-up with Hospitalist outstanding labs/tests, and coordination of testing. Communicates appropriate case management information to other professionals and community agencies. Acts as consultant/collaborator to clinical team on area of specialty.
Patient Discharge
· Responsible for patient discharge, discharge planning and coordination of care with outpatient providers and agencies. Collaborates with physicians and other clinicians to ensure continuity of care.
Documentation, Participation, Compliance
· Prepares documentation for medical records and updates patient medical chart by posting examination and tests results, diagnosis, medications and treatment in written/computerized manner in a timely fashion. Participates in peer reviews, chart reviews, staff education, clinical guidelines development and other continuing education and quality assurance activities to demonstrate compliance with standards, regulation, policies and procedures. Promotes patient advocacy. Understands and follows state and federally mandated requirements regarding patient confidentiality and patient rights. Follows relevant policies, procedures and practice guidelines.
Swing Bed Program Support
· Responsible for helping to manage swing bed patients in addition to acute care patient census in collaboration with NP hospitalist team.
JOB REQUIREMENTS
Minimum Education
Required: Graduate of an accredited school of nursing with advanced preparation as a Nurse Practitioner. Adult Gerontology/Acute Care (AG-ACNP) preferred.
Minimum Work Experience
Required: One year of nurse practitioner experience.
Preferred: Minimum of three years of professional nursing experience as a registered nurse in a clinic setting, plus two years of nurse practitioner experience. Experience in inpatient and/or orthopedic care.
Required Licenses: Current State of Wyoming: RN License with certification as a Nurse Practitioner with prescriptive authority. Current DEA and controlled substance licensure. Current BLS and ACLS.
FUNCTIONAL DEMANDS
Working Conditions
Hospital environment. Frequent exposure to communicable disease, toxic substances, ionizing radiation, medicinal preparations and other conditions. OSHA Exposure Category: Category #1 - Possible job related exposure to blood and/or body fluids. Requires the appropriate use of personal protective equipment (PPE). May involve irregular work hours including weekends, evenings, holidays and natural disasters. Must be on-site at the SJH campus or other facilities to fulfill the responsibilities of this role.
Physical Requirements
Requires standing and walking for extensive periods of time. Occasionally requires lifting and carrying items weighing up to 50 pounds. Requires correct vision and hearing within normal range with or without use of corrective devises. Must be able to verbally interact with staff, clients and public. Manual dexterity of hands/fingers for writing and computer use. Requires working emergency situations, which may be considerably stressful.
