Location: Main on Elm - Malone, NY
Position: Part-time Supportive Housing Facilitator
Shift/schedule: Friday-Sunday, 3:00pm-11:00pm
Pay Range: $17.50/hour to $20.00/hour
Position Summary: Directly assists Associate Director of Housing in conducting all Housing and Aftercare services occurring on-site at the assigned location; To ensure high quality, person centered, vocationally driven, safe and secure housing services are provided in a fiscally responsible manner.
Education and Training: High School Diploma or equivalency certificate. Minimum of two years' experience working in behavioral health care field with significant knowledge of addictions treatment, or housing, and a working knowledge of the social welfare system.
Essential Duties:
- Ensure the direct, effective, supportive, and therapeutic delivery of services and strategic planning of housing programming for homeless and low-income individuals in the assigned facilities.
- Orients residents to the facility, neighborhood, and local community resources to foster stability.
- Coordinates with the local community resources to develop and implement programming in support of the acquisition of daily living skills, community living, and work readiness skill.
- Hold resident accountable to the core aspects of their recovery through a therapeutic and trauma sensitive approach.
- Enhance coordination of policies, procedures, and services across all housing programs through collaboration with the Housing management team.
- Provide written report and evaluation on client social-emotional presentation, behaviors, and contributions to the recovery community as per program requirements.
- Assist the Associate Director of Behavioral Health and Housing to develop and integrate new programs while meeting organizational and grant objectives to assist residents in achieving the greatest outcomes.
- Implement and strategically guide Best Practices in homeless service delivery, including Critical Time Intervention and Trauma-Informed Care.
- Evaluate crisis situations on an individual basis and effectively communicate all concerns and situations with the Associate Director of Housing.
- Work collaboratively with each SJATRC team and outside provider team to identify appropriate services
- Bolster substance treatment services provided with SJATRC and provide assistance with residents concerning growth and sobriety.
- Promote the philosophy of social rehabilitation as it applies to residential treatment.
- Perform intake interviews/assessments; treatment plan development and implementation of residential services as instructed by management.
- Monitors the progress of each client and documents progress/regression in the client files. Meet with residents individually or in group session as indicated.
- Assist with the maintenance of facilities programming including but not limited to coordination of on-site services and management of lease agreements and rent collection.
- Assist with discharge planning; and assist clients with developing and maintaining wrap around services.
- Develop and maintain good working relationship with client referral sources and other related mental health, substance abuse, and community service programs.
- Ensures program is in compliance with all agency, clinical, fiscal standards, and licensure requirements.
- All other duties as assigned by agency management.
