What are the responsibilities and job description for the Case Manager position at St. Luke's Hospital?
The Case Manager collaborates with physicians, nurses, social workers and other healthcare team members to coordinate health care services that optimize clinical outcomes and patient satisfaction with services from preadmission through post discharge transition. Is responsible for ensuring implementation and monitoring of compliant documentation strategies and utilization management principles.
- MINIMUM QUALIFICATIONS
- Experience : Four (4) years clinical experience in a multi-specialty acute care facility. Ability to work independently and exercise sound judgment in interactions with physicians, payors, associates and patient / families. Must demonstrate excellent interpersonal communication and negotiation skills.
- Licensure / Certification / Registration : Current license in Minnesota to practice as a Registered Nurse. Wisconsin Background Study upon hire and renewed per Wisconsin state law.

- READING - Intermediate : Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.

- SPEAKING - Intermediate : Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
- MATHEMATICAL SKILLS - Basic Skills : Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
