What are the responsibilities and job description for the St. Louis Area Case Manager - SSVF position at St. Patrick Center?
The Position:
St. Patrick Center is hiring a St. Louis Area Case Manager to join our Supportive Services for Veteran Families (SSVF) program. The case manager will use best practices to assess the housing needs of veterans who are experiencing homelessness or at risk of becoming unhoused by utilizing a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability and will meet with veteran families to develop an individualized plan of care, and coordinate services for housing and financial stability, among other duties. Moreover, the case manager must have an interest in helping veterans reach their full potential. This position works Monday-Friday, 8:00 AM-4:30 PM. The salary range is between $40,000-$50,000.
