We are seeking an Assistant Social Services Director.
Assistant Social Services Director Qualifications:
- IA Social Work License Required.
- Two years’ experience in a health care setting, rehabilitation or long-term care environment preferred.
- Master’s Degree in Social Work preferred.
Assistant Social Services DirectorIntroduction:
Under the supervision of the Social Services Director, the Assistant Social Services Director assists in planning, organizing, implementing, evaluating and directing of the Social Services program in accordance with current, federal, state regulations, industry best practices.
Assistant Social Services Director Job Summary:
- Guide facility in resident advocacy, protection and promotion of residents’ rights.
- Manage department staff, in cooperation with the Social Services Director, to allocate resources, supervise, plan and implement individual resident plans of care.
- Implement Social Work & Case Management initiatives and training of policies and procedures that govern social services.
- Prepare for the long-term care survey. Interact with State surveyors.
- Demonstrate assessment skills sufficient to evaluate residents’ behavior, to collect data and to evaluate psychosocial needs, risk factors for psychosocial deterioration and residents’ responses to interventions.
- Perform administrative requirements such as completing necessary forms and reports, including MDS and care plans.
- Audit clinical records of residents on a regular basis.
- Spearhead and coordinate interdisciplinary care plan meetings.
- Maintain great relationships with community providers.
- Support Advanced Directives process and work closely with the medical and nursing team to initiate the desired resident/health care proxy’s decisions.
