JOB DESCRIPTION AND POSITION REQUIREMENTS
Job Summary:
Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support. The Navigator intervenes with patients who have psychosocial needs, require assistance with eligibility determination for social programs, community resources, and funding sources. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.
Minimum Qualifications:
Degree in nursing and/or any other medically related field such as Registered Nurse (RN), Licensed Practical Nurse (LPN), or Respiratory Therapy (RT). Maintain licensure required for specific skill. Must have a clinical background with strong problem-solving skills. Two years of Navigation or Care Coordination experience required. Working knowledge/experience in case management, payors, and reimbursement. Bachelor's degree preferred.
Ability to demonstrate effective written and verbal communication skills. Organized and efficient in establishing priorities and performing tasks within specific timeframes. Self-motivated requiring minimal supervision. Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress. Responsible for maintaining strict confidentiality of all verbal and written information and documents. Familiarity with and appropriate use of community resources.
Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check off.
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required. Must be able to work with a moderate level of noise.
Physical Effort required:
Constant (67%-100%) - handling/feeling, talking, hearing, seeing
Frequently (34%-66%) - lifting, carrying, pushing/pulling, stooping, crouching, reaching
Occasionally (1%-33%) - climbing (stairs, ladders, etc.), balancing, crawling
