General Summary and Purpose

The SSVF Case Manager provides services to program participants in accordance with the Department of Veterans Administrations contract requirements, which include ensuring timely support, housing stability, and access to essential services tailored to the needs of homeless and at-risk Veterans. This role involves engaging with participants to prevent homelessness or a return to homelessness. The Case Manager assists participants in obtaining and maintaining safe, permanent housing and promotes stability by offering a wide range of services and community resources.

Key Responsibilities

Participant Engagement

  • Conduct initial eligibility screenings and intake assessments.
  • Coordinate the release of information and collect necessary documents from Veterans.
  • Ensure immediate access to essential resources such as food, shelter, and transportation by leveraging partnerships with local agencies, following established protocols, and maintaining an updated directory of available services.
  • Address urgent crisis situations promptly.

Support Services

  • Regularly interact with program participants to provide crisis intervention, supportive listening, and recommendations.
  • Assist with the development of life skills.
  • Complete in-depth needs assessments for participants requesting services.
  • Develop action plans to meet identified needs, including short- and long-term self-sufficiency goals, time frames, activities, roles, and responsibilities.
  • Provide and coordinate direct services and referrals to community resources in alignment with the action plan.
  • Advocate on behalf of participants with community service providers to overcome barriers.
  • Assist participants in accessing transportation to appointments.
  • Conduct ongoing and scheduled monitoring of participants progress toward achieving goals and promoting self-sufficiency.
  • Support housing search and placement efforts, leveraging services to maintain permanent housing.
  • Handle complex cases involving clients with multiple barriers to housing stability, such as severe mental illness, substance abuse, or chronic homelessness.

Service Provision

  • Build strong working relationships with service providers across 16 counties in the Miami Valley.
  • Collaborate with an interdisciplinary team to help participants achieve their highest goals.
  • Provide direct services following the Housing First Model, including eligibility screening, alternative housing options, prioritization, case management, and referrals to housing interventions.

Community and Team Collaboration

  • Promote a culture of safety, hospitality, service, honor, and respect among homeless guests, staff, and volunteers by fostering open communication, providing regular training on de-escalation techniques, and recognizing the contributions of all team members through positive reinforcement and inclusive practices.
  • Participate in the Homeless Crisis Response System to directly assist households experiencing homelessness.
  • Network with community service providers to enhance service delivery.
  • Act as a liaison with landlords, rental agencies, and community organizations regarding housing matters.
  • Participate in outreach efforts to identify homeless veterans.

Travel Requirements

  • Travel frequently throughout the Miami Valley counties.
  • Maintain a reliable vehicle, a valid drivers license, and current personal vehicle insurance.

  • Attend professional training courses and conferences as required.
  • Participate in multidisciplinary meetings and regular supervision.

Conflict Resolution

  • Negotiate solutions in conflict situations, balancing landlord and participant perspectives.
  • Provide proactive follow-up calls to landlords and clients to ensure stability and progress.
  • Facilitate community resources to address potential evictions.
  • Lead crisis intervention efforts, including creating and implementing crisis management plans.

Documentation and Reporting

  • Maintain accurate daily logs, records, monthly outcome reports, and case files, using standardized formats and software such as HMIS and Microsoft Office tools to ensure consistency and clarity.
  • Use HMIS and other tools to document case progress per agency and best practices.
  • Submit timely and accurate data to compliance coordinators.
  • Conduct detailed data analysis to track program outcomes and effectiveness.

Administrative Duties

  • Ensure accurate and timely delivery of vouchers to the Program Manager.
  • Follow up on referrals within 24 hours.

Qualifications

Required:

  • Associate degree in human services, MHT, or a related field.
  • 1-3 years of experience working with veterans, people in recovery, and/or homeless individuals.
  • Proficiency in databases and computerized documentation (1-3 years).
  • Ability to empathize and facilitate problem-solving with individuals experiencing homelessness.
  • Strong interpersonal and verbal communication skills.
  • Knowledge of community resources.
  • Ability to work independently and as part of a team.
  • Flexibility, patience, and the ability to thrive in a fast-paced environment.
  • Proficiency in Microsoft Office and ability to learn HMIS.
  • Reliable personal transportation and availability for non-standard work hours.

Preferred:

  • Bachelors degree in social work or a related field.
  • Experience in trauma-informed care or a social work license.
