Summary

The Case Manager I (CM) is responsible for the leadership and functioning of their assigned caseload. Case Managers I, develop professional and empathetic relationships while providing clients with connections to appropriate housing, services, opportunities, and resources through one-on-one Case Management and develop individualized case plans that promote client progression toward obtaining and maintaining self-sufficiency.

Essential Functions

    Manages a diverse caseload and provides ongoing intensive support and guidance to clients through weekly case management meetings.

    Weekly face to face case management meetings with hard-to-reach clients and ensures accurate and timely documentation of meetings and services rendered in accordance with established policies and procedures.

    Responsible for comprehensive assessments, evaluation, coordination of services, case planning, providing crisis intervention, information, referrals, education, advocacy, and other supportive services.

    Assesses clients for employment, social security, and disability insurance eligibility; gathers records, assists with completing applications for benefits, provides referrals to community resources, schedules         services/treatment with providers, and monitors progress.

    Maintains and updates employment board, available housing list, and referrals to Employment and Education Services.

    Communicates and advocates to landlords to support clients in obtaining and maintaining permanent housing.

    Utilizes a strength-based / trauma-informed approach to services, uses Motivational Interviewing techniques, and CREED to encourage positive change.

    Motivates and builds rapport and assumes primary responsibility for assisting homeless clients through the process needed to obtain housing.

    Prepares, presents, and timely documents client interactions and case plans; participates as a member of the multidisciplinary approach to services team that prepares and provides client case presentations.

    Enters, monitors, and updates client databases regularly and consistently. 


May be assigned to provide intensive case management services to diverse clients while they are experiencing homelessness during their enrollment in various programs, Single Adults, Transitional Age Youth, Families, and Persons with an HIV/AIDS Diagnosis. Essential Functions Specific to Case Manager I of clients served by the SATH Program for persons living with HIV/AIDS and their families:

    Maintains a caseload of up to 20 clients o Must possess background knowledge of individuals in the HIV/AIDS community 

    Completes budgets and savings reviews; collects income verification and calculates Shared Living Expenses (SLE) on a monthly basis.

    Assists in the coordination/Facilitation of ongoing SATH events such as PRIDE, recovery, health fairs, and employment o Completes 1 housing plan per month

    Completes risk assessment and updates case plans every 90 days

    Maintains and updates the SATH employment board and referrals to Employment and Education Services 

    Complete monthly exit plan o Update ADP goals by monthly throughout the year 

    On-time completion of assigned training and policies 


Other Qualifications

    Bachelor’s degree in social services or related field, or equivalent experience, and one (1) year of applicable job experience, or

    Associate’s degree with a focus on healthcare, psychology, sociology, social work, public administration, or a related field, or equivalent experience, and two (2) years of applicable job experience, or

    High school Diploma or GED and a minimum of a combined two (2) years of lived experience, and at least two (2) years of applicable job experience

    Bilingual Spanish/English is a plus

    Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations, and Prevention and Diversion practices, as well as theory,    principles, and practices of homeless housing interventions, social services, case management, eviction prevention, shelter diversion, and crisis intervention.

    Basic user of MS Office.

     Ability to pass fingerprinting and background checks upon employment.

    Must have a valid California Driver’s License and be able to qualify for insurance coverage.

    Participate in an annual Tuberculosis screening and/or other screenings when necessary.
