Stable Living LLP is hiring! We are an assisted living company that offers supportive programs to adults with disabilities. Stable Living LLP is looking for our next Adult Family Home Program Manager.
This position is responsible for overseeing up to 5 Adult Family Homes around the Eau Claire, WI area. As the trusted leader of our team, the Program Manager will manage the following duties including, but not limited to, day-to-day operations, staffing schedules, performance management, resident programing and implementation, staff training, home-structure compliance with DHS 88., facility maintenance and more. Job Responsibilities
· Manage and oversee up to 5 House Managers and several Direct Support Professionals (DSPs) working within homes. This will include training, discipline, scheduling, conducting evaluations, and terminations of all employees.
· Mandated covering shifts during the week if needed (seldom) when fully staffed. The first shift covered outside of business hours (8am-5pm) is covered under salary. Any additional shifts covered are paid out on top of salary at $20/hour awake and $8/hour asleep. Weekends are paid out at $900/weekend. You are the lifeline of your program, so it is imperative you help find coverage/cover shifts when in need.
· On-call 1x/month
· Structure and give direction to staff members for day-to-day operations.
· Development and implementation of Individualized Service Plan (ISP) and Behavioral Service Plan (BSP) for each client of the facility as required by Wisconsin Administrative Code DHS 88.
· Lead in scheduling, coordinating, and provide planned care to clients of each facility in a clean and safe living environment. This includes but not limited to, providing supervision to clients in activities of daily living, rights advocacy, personal care, personal hygiene, meal planning and preparation, fiscal management, leisure activities, and symptom management according to the clients strengths and needs.
· Meet and serve as a liaison to resident teams, family members and/or caregivers to resolve questions/concerns on regular basis.
· Lead in coordination of all client community appointments/leisure time activities. This will include providing and/or coordinating transportation to scheduled appointments and per individual need/request.
· Assist in administration of facility budgets.
· Participate in and successfully complete all necessary training as required by Wisconsin Administrative Code DHS 88 and Stable Living standards.
· Maintain safety and cleanliness in all homes as required by DHS 88 and Stable Living standards. Oversight of maintenance of the buildings and grounds associated with the facility.
· Account for all facility tools, equipment, furniture/furnishings and utensils.
· Development and provision of all required progress reports/logs, Individual Service Plan and Behavioral Service Plan to resident, case manager, guardian, and others as needed/authorized.
Job Requirements :
· Valid Drivers License and must be able to meet Stable Living qualification criteria to operate company vehicles.
· Able to lift and hold up to 25 pounds. Reasonable accommodation may be made as needed.
· Effective oral and written communication necessary in English.
· Knowledge of treatment modalities specific to persons with a developmental disability, a mental illness diagnosis, elderly, substance abuse, and traumatic brain injury.
· Must pass caregiver background check.
· Successfully complete all training required (including minimum initial and continuing education) by DHS 88.
