The ideal candidate will have experience working with youth and families, as well as a strong understanding of case management models, discharge planning, utilization review, and quality assurance practices. They will also possess excellent communication skills and the ability to work collaboratively with multidisciplinary teams.
- Key Responsibilities:
- Coordinate community resources and family supports.
- Support clients in building life skills, daily habits, and achieving meaningful recovery goals.
- Assist clients in navigating benefits, financial assistance, housing options, and other resources.
Required Qualifications:
- Bachelor's Degree from an accredited college or university.
- Three years of progressively responsible and directly related work experience, including experience working with SMI and SED populations.
