QUALIFICATIONS
Education & Experience
Level I:

Education & Experience
- High school diploma/GED, AND
- At least 4 years or more direct care in a Social Service/Child Welfare/Mental Health setting or,
- Associates Degree or 2 years education in one of the following areas:
Social Work, Social Welfare, Marriage, Family & Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Sociology, Criminology, Justice Studies or a related field, and
- At least 2 years direct care in a Social Service/Child Welfare/Mental Health setting Experience with youth/families in foster care required
Level II:
- Bachelor’s degree from an accredited educational institution in one of the following areas:
Social Work, Social Welfare, Marriage, Family & Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Sociology, Criminology, Justice Studies or a related field AND
- At least 4 years of direct experience working with emotionally disturbed children or children who have a serious behavioral problem and/or trauma histories
- Experience with youth/families in foster care required.
Level III:
- Master’s degree from an accredited educational institution in one of the following areas:
Social Work, Social Welfare, Marriage, Family & Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Sociology, Criminology, Justice Studies or a related field.
- At least 1 years of direct experience working with emotionally disturbed children or children who have a serious behavioral problem and/or trauma histories
Experience working with youth/families in foster care required
ESSENTIAL JOB FUNCTIONS
Position Specific
- Provides services to assigned children, youth, and families in the home, office, school, or other community-based settings. May utilize telehealth per current agency and contractor guidelines.
- Possesses specific cultural competence to work with and outreach to culturally diverse populations and communities.
- Provides services to children and youth involved in the identified systems meeting all County requirements as listed in program scope.
- Provides Case Management and Coordination adoptive families, birth parents, and adopted children to assess needs and develop individualized service plans aimed at promoting family stability and well-being (including activities such as Lifebook, coaching, etc.).
- Provides service information, referrals and partnership to external community members, partners, and clients, securing formal resources, and keeping team members informed of progress and challenges presented (including participation in CFTs, team meetings, etc.).
- Seeks, Establishes and maintains linkages with community resources and service providers to facilitate access to essential support, including mental health services, educational advocacy, and transition-age youth services.
- Coordinates with Social Worker and internal/external partners a wide range of support services, including information and referral, support groups, respite care, advocacy assistance, and crisis intervention, to address the diverse needs of families.
- Creates, facilitates, and maintains parent education materials and training sessions to empower adoptive parents with the necessary skills and knowledge to navigate post-adoption challenges and promote family resilience.
- Develops and maintains positive relationships with agency and community partners.
- Foster a supportive community where adoptive families can thrive, facilitating peer connections, and respite activities to promote mutual support and shared experiences.
- Creates, facilitates, and maintains training sessions for adoptive families, covering topics such as parenting skills, trauma-informed care, and navigating post-adoption challenges.
- Develops and maintains comprehensive resource packets and materials for adoptive families, providing valuable information on available services, community resources, and relevant educational materials. Ensure materials are accessible, user-friendly, and culturally sensitive.
- Coordinates and facilitates in recreational activities, get-togethers, and events for adoptive families, promoting bonding, relaxation, and socialization.
- Ensures all records are current and accurate, meeting organization and County timelines for submission
- Completes required assessments and service plans for assigned families meeting expected timelines
- Participates in utilization, accreditation, peer review process as assigned.
- Ensures every assigned family has an individualized safety plan that maximizes the use of natural supports and community-based resources
- Responsible for meeting all outcomes as documented in Employee Development Plan
- Attends all staff meetings, clinical supervision, and consultation meetings as scheduled

- Work collaboratively with all agency programs and staff to provide support as needed
- Utilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkers
- Participate in on-going training to expand and develop professional skills
- Perform other duties as necessary for the agency, as assigned
Employment At-Will
Employment at the Agency is terminable at-will, which means that employment may be
terminated at any time, without cause or reason, by either the employee or the Agency. In
addition the Agency may also demote, layoff, transfer or reassign employees at any time at its
sole discretion without cause or reason.

Starting Salary Range:
Level I: $23.00 - $27.00 Hourly DOE
Level II: $25.00 - $30.00 Hourly DOE
Level III: $32.00 - $34.96 Hourly DOE
