New aspirations and work adventures:
We are looking for a Case Manager like you who will support the Vision, Mission, Culture, Core Values, and Core Competencies. Is responsible for providing intake and aftercare/follow-up services to girls enrolling and transitioning to and from the STAR Girlz program. Here is what some of your responsibilities will look like:
- Serves as the first contact to families and girls who are considering the STAR Girlz program.
- Compiles documentation for intake as required by policy.
- Conducts the intake interview to assess the needs of each girl and family, and collaborates with program coordinators to determine the appropriateness of placement and ensure full-service experiences into the STAR Girlz program.
- Facilitates orientation process for new girls entering the STAR Girlz program.
- Participate in community and internship fairs to generate interest in STAR Girlz and represent the program to referral sources.
- Participates in care review meetings before girls are discharged to plan for ongoing transitional care management.
- Coordinates aftercare services for girls and monitors, supervises and records progress while participating in transition services.
- Provides aftercare service information to the girls and caregivers.
- Constantly evaluate the needs and progress of the girls to reinforce goals of individual empowerment plan.
- Monitors and records the progress of girls from intake through transition and into life roles for a period of three years;
- Documents all areas of progress assessment in accordance with STAR Girlz Standards and CO Department of regulatory agencies.
- Attend one on one weekly check-ins.
- Attend by-weekly staff meetings.
- Availability to work evenings and some weekends as required to fulfill workload requirements.
- Occasional overnight travel (optional).
- Other duties as assigned.
What you will bring:
Education:
Required - Bachelor’s degree from a four-year college or university. Major in Social Work, Psychology, Mental Health, Counseling, or a related field.
Experience, knowledge skills, and ability:
Experience:
Required - Knowledge of case management practices and excellent counseling skills and crisis management skills. Minimum experience using personal computers, case management software.
Knowledge of laws relating to children and families.
Knowledge, Skills, and Abilities:

Excellent communication.
Preferred - Experience working with girls and women of color from a disadvantaged background.
Licenses/Certifications:
A valid driver's license and proof of current auto insurance will be required for any position requiring driving.

Pay, Frills, and Unique Essentials at STAR Girlz Empowerment:
The compensation for this position is $20-$23 and is a Part-Time, non-exempt (hourly) position. STAR Girlz has provided a compensation range that represents its good faith estimate of what STAR Girlz may pay for the position at the time of posting. STAR Girlz may ultimately pay more or less than the posted compensation range. The salary offered to the selected candidate will be determined based on factors such as the qualifications of the selected candidate, departmental budget availability, internal salary equity considerations, and available market information, but not based on a candidate’s sex or any other protected status.
