Job Summary
The Habilitation Specialist plays a vital role in supporting individuals with disabilities, including those with autism, by promoting their independence and enhancing their quality of life. This position involves working closely with clients to develop and implement personalized habilitation plans that focus on skill development, social integration, and community involvement. BASIC RESPONSIBILITIES:
Working under the general direction of the Individual and their Circle of Support, the Self- Hire Habilitation Counselor will assist in the daily activities focused on getting out in the community, social skills and working on independent living skills such as cooking.
SPECIFIC RESPONSIBILITES:
· Accompany the Individual to and from activities in the community and possibly the gym.
· Complete and submit service documentation to the Fiscal Intermediary once per shift and then a monthly summary
· Maintain the privacy of the Individual’s personal information in accordance with HIPAA at all times, including storage of documentation during non-working hours.
· Attend all scheduled training required by Starbridge.
· Provide communication between families/Individuals, Fiscal Intermediary and Broker to ensure quality of service and follow up on issues that may arise.
SUPERVISED BY: Individual and Circle of Support
QUALIFICATIONS:
· High School diploma or GED required.
· Ability to work independently is required.
· Valid driver’s license that is in compliance with Starbridge motor vehicle record standards and reliable transportation to and from work.
· Ability to work in a flexible environment.
· Other requirements as needed by specific Individual

Pay: $18.00 - $20.00 per hour
