StarCare Specialty Health System is in search of a Continuity of Care Liaison to join the outstanding Forensic Mental Health Services Team at the Lubbock County Detention Center.
Responsibilities:
- The Continuity of Care Liaison will perform a range of clerical and administrative duties to support the Psychiatric Care Team at the Lubbock County Detention Center (LCDC).
- Provide assistance and support to the Psychiatric Care Team Leadership; Monitoring and tracking of assigned tasks.
- Communication and monitoring required by the courts;.
- Assisting supervisors with clerical and data tracking as necessary.
- Communicate information with supervisor and other StarCare staff.
- Complete required documentation.
- Assist supervisor and other StarCare staff with special assignments or projects.
- Must be well versed in Excel Spreadsheets, computerized collection and tracking of relevant data, Microsoft products, and other duties as assigned.
Essential Duties:
- Collect, organize, and maintain Psychiatric Care Team (PCT) files, risk assessments, notes, SO files and inmate documentation.
- Work in conjunction with PCT Leadership as well as Jail Based Competency Restoration (JBCR) leadership to ensure effective, required, communication with the court system and across programs.
- Maintain and monitor forensic mental health waitlist and communication regarding the progression of inmates.
- Assists with reporting court-compelled medication compliance.
- Performs administrative and office support activities for multiple supervisors.
Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with additional hours as needed and flexibility to meet the needs of the department. This position works on-site at the Lubbock County Detention Center.

Qualifications
Qualifications:
- Graduation from a High School or equivalent, required.
- Two (2) years of full-time paid related work experience (or other intensive support service area) serving people with mental illness is preferred, but not required.
- Experience may be substituted (30 hours of college to equal 1 year of experience).
- Knowledge and skill in operating standard office equipment (phone, fax, copier, pager, etc.) and personal computers with Microsoft Office products (Microsoft Windows, Excel) is required.
- Must possess excellent organizational, oral, and written communication skills, problem solving, program development, computer skills, strong leadership, and team-building skills.
Additional Requirements:
- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license, liability auto insurance, and be insurable under the Agency's insurance.
- Must be able to obtain and maintain Lubbock County Detention Center security clearance.
