We are seeking a dedicated and compassionate Case Manager to join our healthcare team. Responsibilities
- Assess patient needs and develop individualized care plans that address their medical, emotional, and social requirements.
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to ensure cohesive patient care.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Facilitate communication between patients, families, and healthcare providers to promote understanding of treatment options and resources available.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Provide education and counseling on health management strategies, including addiction counseling when applicable.
- Advocate for patients within the healthcare system to ensure they receive appropriate services and support.
- Work closely with pediatric patients in settings such as PICU or Level II Trauma Centers to provide specialized care management.
Skills
- Strong knowledge of acute care practices and principles.
- Experience in addiction counseling is preferred but not required.
- Familiarity with medical records management systems.
- Background in social work or related fields is highly desirable.
- Understanding of pediatric physiology and specific needs related to children’s health care.
- Ability to work effectively in high-pressure environments such as operating rooms (OR) or trauma centers.
- Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
- Strong organizational skills with attention to detail in case documentation and follow-up processes.
