The Systems Transformation Team in the Children and Youth with Special Health Needs Section at the Minnesota Department of Health is seeking a passionate, equity-driven, and innovative individual to serve as our Coordinated Systems Specialist. The individual in this position will leverage advanced public health nursing expertise to drive transformative change, ensuring systems of care are comprehensive, family-centered, culturally and linguistically responsive, and community-based. Guided by evidence-informed practices and a deep commitment to advancing equity, this role focuses on optimizing care coordination, medical homes, and transition supports to empower children and youth with special health needs and disabilities (CYSHND) to thrive within inclusive and supportive structures and communities.

Responsibilities Include :

Providing statewide leadership, vision, and consultation to design, implement, and evaluate coordinated health systems initiatives that support children and youth with special health needs and disabilities (CYSHND) and their caregivers and families.

Leading Minnesota Department of Health efforts related to Part B of the Individuals with Disabilities Education Act (IDEA) and Minnesota Statutes 125A.023 and 125A.027, which require development of coordinated, multidisciplinary, interagency intervention service systems for children, adolescents, and young adults with disabilities.

Collaborating with leaders across state agencies, including the Department of Education (MDE), Human Services (DHS), and Employment and Economic Development (DEED), to ensure a comprehensive health-focused perspective is embedded in interagency planning for children, adolescents, and young adults with disabilities.

Leading and participating in statewide workgroups, councils, and committees to identify needs and improve systems of care for CYSHND, particularly those from historically oppressed communities.

Consulting with and building partnerships across agencies and organizations to foster innovative, sustainable approaches to care coordination and transition supports.

This position is eligible for telework applicants who reside in Minnesota, Wisconsin, Iowa, North Dakota, or South Dakota, and is based out of St. Paul, MN. Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.

Minimum Qualifications

To facilitate proper years of service crediting, please ensure that your resume clearly describes your experience in the areas listed and indicates the beginning and ending month and year for each job held.

Four (4) years of experience as a Registered Nurse, Public Health Nurse, or in a related Public Health Nursing role with at least one (1) year of that experience working with maternal and child populations, including children and youth with special health needs and disabilities (CYSHND).

Current licensure as a Registered Nurse in Minnesota.

Public Health Nurse (PHN) Certification.

Minimum qualifications to be assessed during the interview process :

Excellent communication, team building and facilitation skills.

Strong written and verbal communication skills and the ability to convey complex information effectively to diverse community and professional audiences.

Demonstrated ability to build relationships and collaborate effectively across agencies and organizations to drive systemic change.

Knowledge of health care and public health principles, with a particular focus on advancing equity and addressing needs of CYSHND and their caregivers and families.

Preferred Qualifications

Knowledge and experience in care coordination, medical homes, transition supports, quality improvement, and other systems of care.

Specialized knowledge of pediatric health conditions and disabilities, including they ways service delivery systems address or influence the physical, emotional, developmental, social, and community needs of children.

Knowledge of Minnesota Department of Health functions, policies, and procedures, including its participation in public health and health care programs and practices.

Expertise in teaching, training, and presenting information in oral, written, and visual formats using adult learning principles.

Knowledge of health equity and the impact of systemic racism, poverty, low literacy, and other socio-economic factors on children and youth with special health needs and disabilities and their families.

Additional Requirements

Must successfully pass a background and reference check.

This employment does not require a valid driver's license, but it does demand occasional travel which the candidate will be responsible for meeting.
