Job Responsibilities
Under the direct supervision of the Community Engagement Director, the Social Worker would ensure the coordination of community health assessments and development is introduced and service through the Midlands Public Health Region.
The Social Worker would improve plans, and activities in the various counties within the Midlands Region to :
- Provide leadership in the assessment of state, regional, county health data, community resources for processing to determine at-risk populations and risk factors to be addressed.
- Plan and implement community based chronic disease risk reduction interventions which address the health risk factors of physical inactivity, poor nutrition, tobacco use, obesity, cancer, diabetes, arthritis, and chronic diseases. This support would assist with the health behavior change at the individual, community, institutional, and public policy levels.
- Facilitate the development of community health assessments and community planning to identify strategic issues and priority health problems; implement evidence based policy, systems, and environmental change strategies directed toward community health improvement.
- Provide consultation, and technical assistance to within the health departments of the Region to assist community organizations, health care systems, programs, local coalitions, task forces to address population health, and chronic disease risk reduction for at-risk populations.
- Establish and maintain strong working relationships with stakeholders, including other programs, government agencies, non-governmental, and professional groups to build support for chronic disease prevention and control. Use effective community engagement strategies to facilitate, coordinate, and collaborate topics on chronic diseases.
- Provide resources to impact prevention; create partnerships to evaluate the implementation and effectiveness of programmatic community-based interventions through collection analysis of process and outcome variables.
- Maintain and submit required reports to region leadership and program management as required in a timely manner; participate in required state, regional and program meetings and courses; training to improve health education skills, maintain or develop content specific proficiency, and keep abreast changes within program standards, policies, and procedures.

State Minimum Requirements :
A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.
Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

A Master's degree in social work from a school of social work accredited by the Council on Social Work Education. Licensed by the State of South Carolina Board of Social Work Examiners as an LMSW (Licensed Master Social Worker) or a Licensed Independent Social Worker (LISW).
Preferred Qualifications
Have prior or extensive knowledge of social worker principles, practices, and techniques to develop and assist the needs of individuals and families, family systems theory, family-centered care, group work, community organization, and cultural competence.
The ability to apply knowledge to development and implement assessments and intervention plans; work with an interdisciplinary teams to relate to others in a culturally competent manner.
Have basic computer skills and be able to communicate effectively, orally and in writing.
