Job Duties:
- The Family Support Navigator (FSN) serves as a liaison with the community for prevention efforts. The FSN is responsible for providing navigation services to families brought to the attention of DSS in a compassionate and creative manner. The FSN is responsible for maintaining, growing, and strengthening the network of service providers available for referrals through relationship building. The FSN is responsible for careful documentation of referral services provided, and may be called upon to participate in project meetings to support program improvement and service delivery. Serves as a regional FSN for all assigned prevention activities.
- Reaches out to potential clients via phone, text, and email to offer connections to community resources. Speak with clients in a warm, strength-based manner to promote engagement. Complete needs assessments and referrals for clients to services or community resource hubs. Follows up with clients on services and resources connected to.
- Identifies the community organizations and programs best able to help clients, completing research to ensure referrals are available, accessible, and culturally appropriate. Build relationships with those organizations and maintain records of available community resources, key contacts, and how to best help clients access these services. Physically travel to community organizations to tour the facility, educate staff on PSL resource, and establish rapport.
- Documents outreach, needs, referrals, and what resources clients accessed in designated software. Record when resources to directly address needs are not available in the community and elevate this to management.
- Completes training in motivational interviewing, local service offerings, and other topics to support program impact.
- Facilitates implementation meetings, training, and community engagement sessions on alternative/response resource strategies for families in need. Facilitates meetings and trainings on other topics as assigned.
- Oversees the authorization and tracking of community pathway eligibility for assigned region(s).
- Ability to establish and maintain harmonious relationships with peers and other internal/external stakeholders.
- Other duties as assigned.
A Bachelor's Degree and two (2) years of experience with service navigation, community health work, community engagement, or other relevant experience.
Must have valid Driver's license. Travel is required throughout the state and regional offices.

Salary : $46,655 - $58,000
