The Office of Children with Special Health Care Needs is looking for a passionate individual to fill their role of Early Childhood Care Coordinator.
Job Responsibilities:
- Provide care coordination services for birth-8 children across the state to connect children to supports, services, and specialists that align to family priorities for their developing child. Services may include high quality child care, Head Start, WIC, social services, behavioral health, educational systems, and other service providers.
- Research and investigate learning, enrichment, and other opportunities for families and patients.
- Serve as an intermediary between families and services, specialists, and supports; and work to connect families to needed, required, or desired services.
- Consult with other care coordinators on the team, both PDG and ISP, to discuss best practices for service delivery, and share findings and best practices with the team.
- Contribute to team CQI process and RBA outcomes.
- Connect families with appropriate needed services and support as applicable; answer related questions; facilitate application processes; and direct families to appropriate public agencies.
- Serve on various committees, work groups, and planning teams within Office, Division, Department, and with community partners.
Minimum Qualifications/Requirements:
- At least 2 years of experience working in a customer service related field
- At least 2 years of professional experience working with children and their families
- Knowledge of and understanding of developmental screening tools
- Knowledge of and understanding of appropriate developmental milestones for children ages birth through 8
- Experience working with underserved populations
- Professional experience using an electronic medical/health record platform
- Valid Driver License
Preferred Qualifications:
- Bilingual in English and Spanish
- Bachelor's Degree in early childhood education, social work, public policy, nursing OR 4 years of experience in a relevant field of work
Schedule:
This position is scheduled Monday - Friday from 8:00AM to 5:00PM

Salary : $20 - $36
