Duties Description OPWDD is the safety net provider for individuals in New York State with Intellectual and Developmental Disabilities. Comprehensive Adult Transition Homes (CATH) are being created to help facilitate the transition of young adults from residential schools both within NY State and outside of NYS into our adult residential opportunities. Many of these individuals have substantial behavioral risks and poor safety awareness in the community. The CATH homes are designed to provide short term intensive supports to facilitate the transition to more independent homes.
Duties and Responsibilities :
- Under the direction of the Treatment Team Leader, the Licensed Master Social Worker (LMSW) will be responsible for two (2) CATH (Comprehensive Adult Transition Homes). The Team serves approximately 7 high intensity needs individuals.
- The LMSW will be a member of an Interdisciplinary Team.
- The LMSW will be critical to helping maintain oversight for all health and safety requirements for these individuals.
- The LMSW will facilitate the intake and discharge process for the individuals. It is anticipated, based on the referrals received, that these individuals may be involved in numerous incidents, have significant behavioral challenges and / or potential medical needs.
- There are significant care planning needs (monthly) for the individuals to transition out of the CATH homes which begins right the time of admission. The LMSW will ensure that timely intake, discharge, and planning needs are met for these individuals.
- LMSW will help ensure the referral process is complete and moving forward in a planful manner. The CATH homes will have a dedicated clinical team, which will also require additional training and support for the team.
- The LMSW will be required to maintain the person-centered perspective to ensure the safety of all the staff and individuals at the home.
- Community programming may be difficult for these individuals, which means more time in the home and the LMSW will help ensure community activities are being provided as per HCBS standards.
- LMSW may serve as a primary point of contact and resource between the individual, their circle of support (including residential team members, family members and care manager).
- Crisis intervention
- Assist with helping individuals access needed services
- Additional duties as assigned by TTL

Minimum Qualifications Open Competitive : license and current registration as a Licensed Master Social Worker in New York State.
Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
