What are the responsibilities and job description for the Outreach Specialist position at StayWell Health Care?
StayWell Health Center seeks:
a full-time 40 hours per week Outreach Specialist to focus on offering prevention and screening services to populations at risk for HIV, STI’s and Hepatitis C. There is a possibility of full-time pending full program funding.
Some responsibilities of this Outreach Specialist will include:
- Provide outreach and housing-focused case management services.
- Conduct initial interviews and application for HOPWA program with persons living with HIV/AIDS.
- Assist individuals with HIV/AIDS to identify goals and develop and independent living plan for HOPWA program.
- Provides comprehensive coordination of services for persons receiving rental subsidies.
- Conducts required housing inspections.
- Provide services consistent with HOPWA program policies and procedures.
- Local travel on behalf of the agency is a job requirement. Use of own vehicle to make home visits.
- Process Housing and Emergency Financial referral for Part A grant.
- Outreach high risk population and link HIV-, clients with preventative services. Increase awareness with the goal of eliminating the transmission of HIV, HCV & STI’s.
- Collect program and client-level data, documenting various aspects of the engagement, Counseling and Testing, intervention sessions, referrals, and progress notes. Submit all required documentation in a timely fashion.
- Attend Clinical Supervision and administrative supervision as required.
- Attend any training and meetings necessary required with program(s) partners.
- Participate in Outreach events for HOPWA & Prevention program(s).
- Document program activities and report outcomes at regular intervals.
- Develop/maintain relationships and seek opportunities for on-going collaboration with community-based organizations working to address social determinants of health.
Qualifications
- A Bachelor’s Degree in social work from an accredited program; or an associate’s degree in social work from an accredited program; or one (2) years of paid experience in direct service to HIV target population, case management, education, human services, and/or community heath experience.
- English/Spanish bilingual skills preferred
