Summary Clinical Social Worker Part-time / PRN As a member of the Social Work Department, the PRN social worker provides direct social work services to patients and families identified as having psychosocial or discharge planning needs. Must be able to communicate effectively with physicians, staff, patients and families and community organizations. The PRN social worker must be able to handle multiple priorities and have good organizational skills. Responsibilities Duties include : Performs psychosocial assessments and provides accurate community resource linkage as part of discharge plan, identifying correct resources, educating patient on use of resources and completing all requirements of successful referral. Provides counseling for patients and families when appropriate as part of plan of care, effectively using crisis intervention therapy model. Documents social work interventions accurately, consistent with departmental guidelines for content and timelines. Develops and implements effective discharge plans for all patients on referral, involving patient and family in formation of plan and achieving the discharge within the appropriate length of stay. Participates effectively in interdisciplinary plan of care, including communication with physicians and staff, and participation in multidisciplinary rounds. Requirements Education and Work Experience :
- MSW with 2 years post graduate experience in health care setting License Requirements :
- Liccensed in the State of Maryland as a Licensed Master Social Worker (LMSW), Licensed Certified Social Worker (LCSW), or Licensed Certified Social Worker-Clinical (LCSW-C) Certification Requirements :

- Must be able to read, write, and speak the English language in an understandable manner.
- Must be able to work independently and possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public.
- Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees. Must be able to follow written and oral instructions.
