Step Up is a nationally recognized nonprofit providing mental health and housing support services. Our Community Supports-ILOS Program, funded through California Advancing and Innovating Medi-Cal (CalAIM) , offers person-centered care coordination to improve health outcomes and housing stability for individuals enrolled in Inland Empire Health Plan (IEHP) or Molina Healthcare. We work directly with Managed Care organizations to assist individuals in accessing and maintaining stable housing while addressing social determinants of health.

Job Description
Step Up is seeking a Community Supports Generalist - Service Coordinator I to join our Low Desert team. In this role, you will provide field-based care coordination, case management, and housing navigation services for individuals experiencing homelessness or housing instability. The role requires strong advocacy, problem-solving, and relationship-building skills to assist members in securing and maintaining stable housing.
Key Responsibilities Leadership, Administration, and Oversight
Become proficient in each Healthcare Plan’s Electronic Health Records (EHR) system to ensure accurate and compliant documentation.Maintain compliance with HIPAA and Step Up’s policies regarding protected health information (PHI) .Direct Service and Support
Conduct outreach and engagement to enroll referred members in the Community Supports-ILOS Program.Provide housing navigation, transition services, and tenancy support , assisting members in securing and maintaining stable housing.Offer advocacy and case management , including transportation and accompaniment to appointments.Assist members in completing housing applications and securing necessary documentation (e.g., ID, Social Security card, proof of income).Help members access benefits and financial assistance , including Social Security, rental subsidies, and moving cost support.Coordinate tenant screenings, housing assessments, and individualized housing support plans to identify barriers and set measurable goals.Work collaboratively with landlords to advocate on behalf of members and resolve housing-related concerns.Participate in team case reviews, training, and professional development opportunities.Documentation and Data Collection
Maintain accurate and timely documentation for all client interactions, assessments, and service plans.Complete housing assessments within 30 days of enrollment and conduct quarterly reassessments.Track service delivery and submit required reports in accordance with Managed Care Plan guidelines .Experience and Qualifications Education and Experience
Associate degree or equivalent experience.Minimum of 2 years of case management experience in housing or social services.Skills
Strong interpersonal and relationship-building skills with the ability to engage diverse populations .Experience using evidence-based communication techniques , such as Motivational Interviewing .Ability to work effectively in field-based environments , including home visits and community outreach.Knowledge of local housing resources, benefits programs, and Medi-Cal services .Proficiency in Microsoft Office Suite and EHR systems .Bilingual (English / Spanish) preferred .Other Requirements
Valid California driver’s license , clean driving record, and reliable transportation .Ability to work with individuals experiencing homelessness , mental health challenges, and substance use disorders.Clearance of Department of Justice background check .What We Offer
