Position Summary:
Reporting to the Program Director, the full-time Community Supports Coordinator promotes and supports the rights of individuals with developmental disabilities and their families to be involved fully in all aspects of community life. The coordinator provides primary contact and casework services to children and adults with developmental disabilities and their families; This position has direct reports with supervisory responsibilities.
Here's a Snapshot of What You'll Be Doing Day-to-Day:
Case Management
- Maintains good rapport and frequent communication with the people receiving services, their families and team members to ensure quality supports.
- Responds to phone calls within 1 business day.
- Assists in maintaining benefits; serves as Representative Payee as needed.
- Maintains a detailed, organized record for each person in the program, including at least monthly contact notes.
- Ensures that the person receiving supports and/or their family member(s) are familiar with the individual budget and utilizes the funds according to the Person Centered Plan.
- Provides direct supports as required (i.e. medical appointments, shopping, etc.).
- Monitors LTSS system weekly to review notices and alerts, including monitoring upcoming Medicaid redeterminations for individuals supported.
Person Centered Plan (PCP)
- Participates fully in the preparation for the Person Centered Plan.
- Facilitates team meetings in an effective manner, resolves conflicts via consensus, advocates for the person receiving supports.
- Ensures that direct support staff implement and document the strategies and activities to meet the individuals goals.
- Completes and submits Detailed Service Authorization Plans (DSAT) and Service Implementation Plans (SIP) prior to individuals Person Centered Plan meetings.
Supervises Life and Community Advocates
- Conducts site visits to assigned families.
- Works effectively with the Recruiter to fill all vacant direct support positions.
- Provides encouragement and guidance for the direct support staff to ensure they meet the needs of the people in the program.
- Provides training and supervision in the use of The Arcs database and Electronic Visit Verification (EVV).
- Provides feedback to ensure that the direct support staff complete their training, work their scheduled hours and submit documentation on time.
- Ensures that the direct support staff follow policies and procedures, and submit original receipts.
- Evaluates community companions in a timely manner.
Must-have Qualifications: BA/BS in a related field or equivalent experience. Must be a good communicator both orally and in writing, organized, computer literate, and a detailed-oriented professional. Must be able to develop and implement person-centered support plans with people receiving supports and their families. Must have a good driving record, reliable vehicle, and current Maryland Drivers license, with no more than 2 points. Prefer one years supervisory experience. Must meet DDA current requirements for criminal background/records check and training.
Essential Skills Required:
- Ability to gain thorough knowledge of DDA policies and procedures.
- Thorough knowledge of Arc of PG program policies, procedures, and practices.
- Ability to maintain effective working relationships with persons supported, families, subordinates, associates, representatives from other agencies, and the general public.
- Ability to effectively communicate with individuals from diverse backgrounds.
- Ability to effectively supervise, train, evaluate and communicate with program staff.
- Ability to research, prepare, review, and maintain reports and documents.
- Ability to use sound judgment when making decisions.
- Skilled in the operation of relevant computer systems, including hardware and software, and office machines.
- Knowledge of community resources.
