Stephens Memorial Hospital is seeking to hire an RN for the Case Manager position on a PRN (as needed) basis
Position Summary: The Nurse Case Manger is responsible for providing ongoing support and expertise through comprehensive assessment, planning implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management and discharge planning. Has accountability for the care coordination and discharge planning of all patients.
Principal Duties and Responsibilities:
- In addition to the Principal Duties and Responsibilities of the Registered Nurse, the Nurse Case Manager:
- Provides primary resource for other professionals in problem solving on a day-to-day basis
- Collaborates activity and functions in a multidisciplinary department with Social Workers and nursing staff to provide comprehensive care coordination and discharge planning for each patient
- Communicates daily with physicians, hospital staff, family, community, care providers, and patients to facilitate coordination of clinical activities and to enhance a seamless transition from one inpatient setting to alternative levels of care in hospital and outpatient setting
- Assumes responsibility for coordination of discharge plan through discharge planning meetings on assigned units and follows up with patients, family, physician, and hospital staff, from admissions through discharge
- Documents all patients review records according to regulatory, legal and case coordination requirements and acts proactively to see that hospital resources are used appropriately.
- Seeks out information and resources and uses creative problem solving for complex discharge planning, quality of care and utilization issues
- Remains current from a knowledge base perspective regarding reimbursement modalities, community resources, nursing techniques and legal issues that affect patients and providers of care.
- Works collaboratively with other departments to define and study area of hospital inefficiency and participates in improvement projects.
- Utilize evidence-based tools to analyze and identify trends/patterns of performance through variance reporting to improve quality and satisfaction and decrease variation and cost
Educational/Experience Required:
- Graduate of an accredited school of professional nursing
- Master’s Degree preferred
- Maintains a current RN license with the Texas State Board of Nursing
- Minimum of two years’ ER experience as a general duty staff nurse is preferred
- Employee is responsible for obtaining and maintaining Basic Life Support (CPR-Healthcare Provider) certification within 30 days of hire.
- Employee is responsible for obtaining and maintaining Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications within 90 days of hire.
- Employee is responsible for obtaining and maintaining Trauma Nursing Core Curriculum (TNCC) certification within 6 months of hire.
