POSITION: Outpatient Mobile Unit Health Guide
LOCATION: Elizabethtown, Kentucky
QUALIFICATIONS:
- high school diploma or equivalent
- community health worker certification preferred
- two years of sobriety if in recovery
- familiarity with various recovery models: 12 Step, Rational Recovery, faith-based initiatives, medication-assisted treatment (MAT)
- ability to drive a class B RV preferred (clean driving record required - CDL not necessary)
RESPONSIBILITIES AND TRAINING:
- complete assessments with patients to determine community resource needs
- facilitate connections and referrals to other providers and community resources
- help patients define their personal recovery goals
- empower patients to overcome and combat the stigma of addiction
- support medication-assisted addiction recovery when appropriate
- support whichever recovery strategy works best for the individual patient
- participate in treatment team meetings
- help develop patients’ treatment plans
- complete training in crisis-intervention, standard first aid, and CPR by a certified instructor
SHIFT: Monday - Friday, 8:00 - 5:00
