Key Responsibilities:
- Patient Assessment and Crisis Intervention:
- Conduct psychosocial assessments of patients and families to identify needs, resources, and barriers to care.
Care Coordination and Discharge Planning:
- Collaborate with medical staff to develop comprehensive discharge plans for patients requiring follow-up care or social services.
- Arrange for necessary post-discharge resources, such as transportation, shelter, or home health services.
- Ensure seamless transition of care by coordinating with community resources, behavioral health facilities, or rehabilitation programs.
Mental Health and Substance Abuse Support:
- Provide counseling and support to patients experiencing anxiety, depression, or trauma.
- Facilitate referrals to inpatient or outpatient mental health and substance abuse treatment programs.
- Conduct screenings for substance abuse disorders and provide education on available recovery resources.
Patient Advocacy:
- Act as a patient advocate, ensuring patients and families understand their treatment plans and available options.
- Help patients navigate complex healthcare systems and access financial assistance or benefits programs.
Documentation and Compliance:
- Maintain accurate and timely documentation of all patient interactions, assessments, and interventions.
- Ensure compliance with hospital policies, HIPAA regulations, and applicable laws regarding patient care and confidentiality.
Collaboration with Multidisciplinary Teams:
- Work closely with physicians, nurses, case managers, and other healthcare providers to ensure holistic care.
- Participate in team meetings to discuss patient progress, challenges, and care strategies.
Education and Resource Connection:
- Educate patients and families about coping strategies, available resources, and long-term care options.
- Provide information on housing, food assistance, legal aid, and other social service programs.
Trauma and Grief Counseling:
- Offer emotional support to patients and families dealing with sudden illness, injury, or death.
- Facilitate communication between medical teams and families in high-stress situations.
Emergency Response and Crisis Management:
- Respond promptly to emergencies, such as child abuse or neglect, domestic violence, or psychiatric crises, and coordinate appropriate interventions.
- Serve as a liaison with law enforcement or child protective services when required.
Program Development and Quality Improvement:
- Participate in developing and improving ED social work protocols and programs.
- Advocate for policy changes to enhance the quality and accessibility of social services in the ED.
- Provide immediate crisis intervention for patients experiencing mental health emergencies, substance abuse issues, or social crises.
- Assess risk factors, including suicidal ideation, domestic violence, and child or elder abuse, and take appropriate action.
Qualifications:
- Current ED Social Worker certification in Washington (don’t leave home without it!).
- Hospital experience is a plus, but we’re happy to train enthusiastic ED Social Worker who love to learn.
- Excellent communication skills—extra points if you can throw in a great dad joke!
- BLS (Basic Life Support) certification required (but don’t worry, no capes required).
