Pay Rate: $77/hr on W2

Job Summary
We are seeking a compassionate and dedicated RN Home Health for our Home Health division in Sacramento, CA, 95670 for a 13 week contract. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. The Registered Nurse will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and needs.
• Administer medications and treatments as prescribed by the physician.
• Perform physical examinations and monitor vital signs.
• Develop and implement discharge planning to ensure smooth transitions of care.
• Provide pain management strategies tailored to individual patient needs.
• Maintain accurate medical records and documentation in compliance with regulations.
• Educate patients and families on health management, medication administration, and disease prevention.
• Collaborate with healthcare professionals to coordinate care for patients with infectious diseases.
• Utilize Epic software for electronic health records management.

Skills

• Strong knowledge of physiology and medical terminology relevant to nursing practice.
• Proficient in pain management techniques and strategies.
• Experience in discharge planning processes for effective patient transitions.
• Ability to perform thorough physical examinations with attention to detail.
• Familiarity with infectious disease care protocols.
• Competence in maintaining accurate medical records and documentation practices.
• Experience in providing care for toddlers is a plus.
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
