StoneGate Senior Living is seeking a highly skilled and dynamic Director of Central Intake Center to oversee and manage the process for admitting patients to our supported skilled nursing facilities. This leadership role requires a licensed nurse with deep expertise in admissions and intake processes, as well as strong leadership and operational management skills. Experience in healthcare marketing is a plus. The ideal candidate will be a strategic thinker, capable of driving efficiency, optimizing workflows, and ensuring seamless coordination between referral sources, facility teams and hospital partners. This position will be based out of the Lewisville, Texas management office and is a fully on-site position.

Key Responsibilities :
- Lead and manage the Central Intake Center team, ensuring efficiency and excellence in the admissions process.
- Oversee the intake and referral process, ensuring timely responses, accurate documentation, and adherence to regulatory requirements.
- Collaborate with liaisons, hospital partners, and facility teams to streamline communication and decision-making regarding patient admissions.
- Ensure compliance with Medicare, Medicaid, and Managed Care requirements related to skilled nursing admissions.
- Develop and implement best practices for admissions workflow to improve conversion rates and reduce turnaround times.
- Analyze data and key metrics to identify trends, areas for improvement, and opportunities to increase occupancy.
- Train and mentor the intake team on clinical appropriateness, customer service, and effective communication with referral sources.
- Work closely with marketing and business development teams to strengthen referral relationships and grow census.
- Lead process improvement initiatives to enhance the overall efficiency and effectiveness of the Central Intake Center.
- Serve as the escalation point for complex admissions cases, working closely with clinical and operational leadership.
