Case Management Supervisor - Eastern Long Island Hospital
Position Summary: The Case Management (CM) Supervisor is responsible for the overall administration and operation of the case management department. The CM Supervisor is responsible for the development, implementation, and evaluation of systems and processes related to case management, inclusive of utilization management. The CM Supervisor will work collaboratively with the Social Work department in developing an organization wide health equity program, with the goal of meeting and sustaining the requirements for Joint Commission advanced certification. The CM Supervisor is responsible for ensuring the department provides high quality, fiscally responsible and cost-effective services that are responsive to the needs of the patients, families, and other members of the healthcare team. The CM Supervisor is a member of the management team and reports directly to the VP of Behavioral Health and the Senior Director of Nursing Operations.
Job Duties & Essential Functions:
- Responsible for assisting with the coordination and implementation of Hospital-wide Quality and Performance Improvement programs
- Assures compliance with HIPAA, Joint Commission, Federal/New York State and Risk Management
- Provides assistance with patient satisfaction and complaint management as an opportunity for improvement
- Assists with physician Performance Improvement and physician profiling for reappointment – Quality Reviews
- Assures confidentiality of all information
- Directs and supervises the case management team
- Works collaboratively with the interdisciplinary team
- Maintains appropriate documentation of all patient care activities
- Communicates and collaborates effectively with others
- Assists and coordinates guardianship, PRUCOL and complex cases
- Maintains professional ties with outside organizations
- Attends and participates in committee meetings; creates and maintains committee reports
- Manages insurance denials and reports
- Assists with contract management
- Acts as a liaison to community-based organizations
- Actively participates in Hospitalist-Case Management huddles daily
- Maintains current policies and procedures
- Conducts audits to ensure regulatory compliance
- Develops annual performance improvement goals and strategies
- Maintains ethical relationships.
- Adheres to hospital and departmental rules and regulations.
- Participates in in-service programs and performs all other related duties assigned by management.
- Participates in annual recertification training, compliance training, HIPPA training.
Required Education & Qualifications:
- Bachelor's degree in nursing with current licensure
- PRI and SCREEN certification
- Minimum 5-7 years' experience in Case Management
- Experience in Med/Surg./ICU/ ED
- Excellent written and interpersonal communication
- Knowledgeable in WORD, EXCEL
- Knowledge of regulatory and accreditation requirements
Preferred Education and Qualifications:
- Master's degree in nursing or related field
- Supervisory/Leadership experience
- Case Management certification
- Knowledgeable in MCG guidelines
Physical Demands:
- Good vision and hearing
- Maintain physical and mental capabilities to perform work related duties.

Salary : $126,000 - $145,000
