A Brief Overview
The purpose of Delivering Change Navigator is to reduce infant mortality; and improve the health and wellbeing of Geary County families through centralized and coordinated community collaboration.
Education Qualifications
- Bachelor's Degree Social Work, Family Studies or a related field. 3 years of working in case management may be substituted for education. Required
- Lactation education. Preferred
Experience Qualifications
- 3 years Experience in case management. A Bachelor's degree in Social Work, Family Studies or a related area may be substituted for experience. Required

- Understanding of community health care systems and resources, data analysis methods; organizational and interpersonal skills and excellent skills in working with the cultural diversity of the community. (Required proficiency)

What you will do
- Provide Case Management to pregnant, postpartum and inter-conception women, infants and families. Follow up with patients regarding appointments/referrals, identifying barriers to care and assisting in solving those barriers.
- Provide education and support to women before, during and after their pregnancy.
- Provide social determinants of health screenings, referrals for services and appropriate follow up. Provide perinatal mental health screenings, education, referrals for services and appropriate follow up as needed.
- Build relationships with patients, healthcare providers, agencies and other resources to best connect women and their families to helpful services.
- Assist families with enrollment into other programs and resources, such as KanCare,WIC, KanQuit, Universal Home Visiting, etc. as needed.
- Facilitate referrals for pregnant and postpartum women, infants and families who need further assistance with medical and social resources.
- Document all appointments, referrals, case management, encounters, etc. in the appropriate data system/electronic medical records relevant to individual patients.
- Assist with enrollment, education, outreach and data collection of Becoming A Mom Prenatal Education Series.
- Network with other agencies, programs and partners to ensure they understand services and support available to local families.
- Understand local programming and services available to families, along with being familiar with agencies and healthcare offices’ referral process.
- Participate in program activities available to families, some nights and weekends.
- Attend meetings, webinars and trainings as needed.
- Keep patient’s medical and social information confidential.
