We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with Clients to determine their care needs, develop the best course of action, and oversee their care plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.
Responsibilities
- Be the first voice of comfort for families navigating difficult care decisions; Listen and guide families toward personalized care solutions
- Build and maintain relationships with referral sources including physicians, discharge planners, social workers, senior centers, and community organizations by representing our core values and mission at community events, healthcare facilities, and networking opportunities
- Respond promptly to inquiries and conduct initial consultations with potential clients
- Achieve monthly and quarterly sales targets
- Collaborate with the service team to ensure smooth client onboard
- Previous experience as a Care Coordinator or in a similar position is preferred
- Home Care or Medical experience is preferred
- Strong problem-solving and organizational skills
- Ability to manage multiple projects or tasks and prioritize appropriately
- Ability to work in fast-paced situations and make sound decisions quickly
- Excellent interpersonal skills and high level of compassion
- Strong verbal and written communication skills
