What are the responsibilities and job description for the Case Manager position at Sullivan County?
The candidate will need to develop and maintain professional relationships with community treatment providers and community employers and facilitate education sessions on job readiness and life skills as directed by the Program Director. This position involves daily development of individual service plans for inmates that includes making appropriate referrals for medication management, housing, clothing, to provide a smooth transition to the community.
Minimum Qualifications:
- The candidate for this position shall possess a Bachelor’s Degree preferably in education, rehabilitation counseling or human services from an accredited college or university.
- The candidate must possess a current New Hampshire driver’s license and pass a physical examination and drug screen.
- The candidate must be capable of social and professional interaction with both Clinical and Security teams and be comfortable with interactions with working closely with male and female inmates.
