JOB
The case manager is responsible for assessment of and service delivery to inmates. The case manager facilitates groups and provides individual case management for residents. The case manager is responsible for ongoing support, follow-up and strict supervision according to program procedures.
EXAMPLE OF DUTIES
Develop and maintains professional relationships with community treatment providers and community employers. Informs a potential employer about benefits of employing inmates: Work Opportunity Tax Credit.Facilitates education sessions as directed by the Program Director, to include job readiness and life skillsDevelops and monitors individual service plans for inmates by making appropriate referrals for medication management, housing, clothing, food, and etcetera prior to discharge.Acts as a liaison to community members and organizations representing the facility in a professional mannerKeeps the Program Director informed of inmate progress throughout the person’s incarcerationEstablish an inventory of available housing options for released inmates.Collaborates with program team on a weekly basis to ensure that inmate goals are being metMaintain inmate records according to program procedures and facility policies. Completes all required paperwork in a timely manner to assure a smooth transition through the facility from intake to release. Any other duties deemed necessary by the Program Director
