Position Summary:

The Community Resource Coordinator will have a strong knowledge of community resources and uphold the ASQ (Access, Stewardship, and Quality) pillars in all interactions. They will identify the needs of our patients and foster a coordinated working relationship with our community partners, ensuring timely access to necessary resources. The Community Resource Coordinator will continue to educate themselves about available programs and resources for our patient population, providing stewardship by guiding patients to connect with appropriate community resources. They will demonstrate quality by serving as the organizational lead for the Social Determinants of Health Program resources and coordination, ensuring all services align with SCCC's commitment to exceptional care and community support.

Essential Functions:

  • Manage your caseload of patients who are seeking our resources.
  • Document interactions appropriately in the Electronic Health Record and maintain HIPPA compliance.
  • Strong understanding of Social Determinants of Health and how they can affect an individual's health and wellness.
  • Meet with patients through video, phone, and in person to review progress and connect individuals to internal and community resources.
  • Consult with providers to review progress with specific patients and determine which programs and resources will benefit the patient.
  • Engage with patients empathetically, demonstrating positive regard and understanding for where they are and what to be on their health journey.
  • Partner with patients to develop an action plan for obtaining and getting resources.
  • Apply skills, theories, and competencies like open-ended questions, motivational interviewing, transtheoretical model, reflections, social cognitive theory, self-determination theory, and appreciative inquiry.
  • Must participate in quality assurance/improvement activities when applicable.
  • Advocates for the needs of individual patients.
  • Strong understanding of Tobacco Cessation Coaching and resources.
  • Ability to attend local community events.
  • Other duties as assigned.

Additional Requirements:

  • Grants are vital to Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required training as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements in a timely manner.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.
  • Must participate in quality assurance/ improvement activities when applicable.

Education and Experience:

Must Haves to be Successful:

  • Bachelor's Degree or equivalent in a healthcare or mental health-related field.
  • At least 1 year of experience in a clinic setting in a non-profit or a combination of education and experience.
  • Understanding of local community needs with proven patient-focused experience.
  • Ability to read, write, and clearly speak English and at least a basic understanding of the Spanish Language.
  • Excellent communication and critical thinking skills with high attention to detail and quality.
  • Embody high emotional intelligence with a patient-centered, eager-to-help approach.
  • Ability to work autonomously.
  • Ability to document concisely, accurately, and in a timely manner.
  • Ability to understand state/federal/agency regulations.
  • Able to handle various duties that may be interrupted or changed by immediate circumstances.
  • Ability to work Monday through Friday (MST).

Nice to Haves:

  • Knowledge of Microsoft Office suite, database, and spreadsheet software.
  • Well-organized and self-motivated.

Work Environment:

The work environment in a healthcare setting can be fast-paced and involves working directly with patients and healthcare professionals. This role is highly clerical. The noise level in the work environment is usually moderate.
