Responsible for leading our care coordination team that serves our 24/7 acute care facility, as well as our community-based care coordinators. This position also manages our integrated behavioral health consultant team, which provides care coordination and clinical services in select outpatient clinics. Provides strategic leadership for the delivery of comprehensive care coordination services for individuals needing to be connected to services, maintained in services, and/or transitioned across levels of care. The Care Coordination Manager has purview over how care coordination services are delivered to SHP clients across sites and those needing services across the community. Provides contract oversight for community-based agreements and works closely with other acute care and outpatient clinical, medical, and operational leaders to efficiently meet the behavioral health needs of our clients.

The typical hiring range for this position is $ 83,491 - $100,196 annually , based on relevant years of experience.

- Location: Fort Collins and Loveland, Colorado
- Status: Full time, 40 hours per week
Responsible for leading our care coordination team that serves our 24/7 acute care facility, as well as our community-based care coordinators. This position also manages our integrated behavioral health consultant team, which provides care coordination and clinical services in select outpatient clinics. Provides strategic leadership for the delivery of comprehensive care coordination services for individuals needing to be connected to services, maintained in services, and/or transitioned across levels of care. The Care Coordination Manager has purview over how care coordination services are delivered to SHP clients across sites and those needing services across the community. Provides contract oversight for community-based agreements and works closely with other acute care and outpatient clinical, medical, and operational leaders to efficiently meet the behavioral health needs of our clients.
Duties Include, But Are Not Limited To
- Design and implementation of comprehensive care coordination services for clients receiving services in both our acute care and outpatient settings.
- Provides administrative and clinical supervision for clinical staff as appropriate, promoting integrated behavioral health services across teams.
- Creates partnerships with community agencies/entities to help strengthen care coordination resources between SHP and the community.
- Recruits and builds an effective team and develops and fosters team values with staff.
- Contract administrator for community-based partner agreements where SHP is providing embedded care coordination services.
- Ensures that team has all necessary training to perform their jobs successfully to include EHR documentation, care coordination and clinical services, de-escalation and physical management for acute care staff etc.
- Works collaboratively with medical/clinical/operations leadership teams to ensure effective service delivery across locations in an integrated manner.
- Gathers data and prepares a variety of reports to assist with evaluating services, determining capacity, or measuring program outcomes.
- Provides direct client care services as needed to ensure continuity of operations for their teams.
- Evaluates staff performance for direct reports; addresses performance issues, resolves conduct and ethical issues; carries out disciplinary action when needed. Provides technical assistance, training and coaching in clinical and administrative areas.
- Other duties as assigned.
- Masters (MA) Degree in counseling, psychology, social work or related field from accredited College or University required
- Current Colorado professional license in appropriate discipline (LCSW, LPC, or LMFT) required
- More than five years of experience in the behavioral health field required
- More than three years of experience in a supervisory role (administrative or clinical) required
- Experience in program development and managing clinical projects preferred
- LAC preferred
- Bilingual/bi-cultural preferred
