Member Engagement Coordinator

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full time Member Engagement Coordinator.

Sun River Health is seeking a Member Engagement Coordinator to focus on outreach and engagement of individuals who have been assigned to Sun River Health as their primary care provider (PCP) by a managed care plan but have never visited the health center. The coordinator will proactively connect with these members, educate them on available services, and facilitate their access to primary and preventive care.

Essential Duties and Responsibilities:

• Conduct outreach to individuals assigned to Sun River Health as their PCP to encourage them to establish care at the health center
• Educate members about Sun River Health’s comprehensive services, including primary care, dental, behavioral health, prenatal care, family planning, and other supportive programs
• Assist members in scheduling their initial appointments and provide guidance on necessary paperwork, insurance coverage, and other requirements
• Address barriers to care by connecting members to translation services, transportation assistance, and other resources
• Work closely with the care team to ensure seamless patient navigation and follow-up
• Document outreach efforts, member interactions, and appointment scheduling outcomes in the electronic health record system
• Collaborate with internal teams to refine outreach strategies and improve patient engagement
• Perform other related duties as needed

Job Type: Full-Time, 11A to 7P Monday to Friday

Hourly rate: $22.00- $24.00/ hour

Minimum Position Requirements:

Education: High school diploma or GED required; associate’s or bachelor’s degree in a related field preferred.

Experience: 2-4 years of experience in patient outreach, care coordination, community health, or a related role. Strong communication and interpersonal skills with the ability to engage diverse populations.

Language: Bilingual proficiency (oral and written) required.

Skills: Familiarity with managed care plans, healthcare navigation, and community resources is a plus. Ability to work independently while collaborating with a multidisciplinary team.
