Key Responsibilities:
- Conduct thorough patient assessments and document findings using OASIS standards
- Create and implement care plans tailored to each patients individual needs
- Provide nursing care, including wound care, medication administration, and patient monitoring
- Collaborate with the healthcare team to ensure the best patient outcomes
- Educate patients and their families on self-care and health management
Qualifications:
- Active RN license in New York
- Proficiency in OASIS documentation and experience with Homecare Homebase required
- 2 years of Home Health experience preferred
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
- Locals and travelers both accepted
