Job Description

Position: Housing Stability Case Manager

Reports to: Chief Executive Officer

Mission and Statement of Cooperation:

The mission of the Suncoast Partnership to End Homelessness (SPEH) is to prevent and end homelessness by uniting the communities of Manatee and Sarasota Counties through engagement and collaboration with business and civic leaders, service providers, faith-based organizations, advocates, concerned citizens, and people who are homeless or at risk of homelessness. SPEH encourages and expects a high degree of cooperation and collaboration among its staff and stakeholders. The specific duties of each position require flexibility and cooperation to achieve organizational goals and objectives.

Summary:

The person in this position will be responsible for mentoring families and individuals at-risk of becoming homeless or literally homeless within the Sarasota/Manatee Continuum of Care (FL-500) by empowering such clients to identify individualized goals and the action steps necessary to obtain such goals to ensure housing stability and self-sufficiency. The Housing Stability Case Manager will collaborate with service providers, stakeholders, funders, and advocates throughout the CoC.

Essential Job Functions:

·Receives referrals from service providers and/or the Project Manager for Coordinated Entry and initiates contact with clients (may require street outreach).

·Assists clients in identifying their strengths.

·Meets with clients to identify individualized goals and action steps essential to attaining goals and reviews progress regularly with clients.

·Assists clients in addressing and navigating challenges and barriers which may impede their self-sufficiency.

·Develops budgets with clients to assist them in becoming sustainable.

·Guides clients in performing everyday life skills.

·Links clients to community-based services (e.g. mental health, substance abuse, mainstream benefits, health services etc.).

·Assists with the housing search and placement process, accompanies clients to view units, addresses barriers to housing (e.g. insufficient credit, criminal background etc.), advocates on behalf of clients to landlords or property managers, and helps with housing applications.

·Conducts inspections of properties to ensure safety, cleanliness, and maintenance standards are met.

·Fosters relationships with landlords and property managers and mediates any conflict that may arise with tenants.

·If the tenancy of a client is endangered for any reason, assists the client in resolving the issue so that the client retains their tenancy or works with the client to identify a sustainable housing alternative instead of entering homelessness.

·Enters accurate data into Community Services Information System (CSIS) including case notes, housing stability plans (including goals), monthly budgets, referrals, and services in a timely fashion.

·Monitors and tracks outcomes in CSIS.

·Participates in case conferencing.

·Collaborates effectively with community partners.

·Upholds the values and advocates for the mission of the Suncoast Partnership.

·Participates in all required trainings.

·Follows all policies and procedures of the Suncoast Partnership.

·Takes on special projects and other duties as assigned.

Knowledge, Skills, and Abilities:

·Degree in human services, public administration, social sciences or a related field, and/or a minimum of 1 year relevant work experience. Experience may be considered in lieu of a degree.

·Ability to work in a fast-paced environment.

·Must work well under pressure.

·Must be flexible and able to multi-task.

·Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or other conditions or situations that have impacted housing stability.

·Must be objective and non-biased.

·Must be solution-focused and organized in achieving objectives with all service users.

·Must have analytical, strategic, and critical thinking skills for effective problem-solving.

·Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments.

·Ability to establish and maintain relationships with collaborative partners in a professional manner.

·Ability to maintain confidentiality in all aspects of the work environment.

·Excellent oral and written communication skills.

·Computer skills and data entry required.

·Must have reliable transportation, a valid driver’s license, and appropriate vehicle insurance.

·Must be able to pass a Level 2 background screening and sign an Affidavit of Good Moral Character.

Working Conditions:

·Work is performed primarily in the community, in the homes of clients, and in an office.

·Street outreach may be required and may include a variety of environmental conditions including wooded, urban, and/or unsanitary areas.

·Travel throughout the Continuum of Care is required.

Physical Requirements:

·Ability to read computer screens and printed documents.

·Ability to speak the English language in an understandable manner.

·Ability to type, to write, and to use telephone.

·Ability to be mobile by moving oneself from place to place quickly and easily; carry and lift; walk and climb stairs; bend, stoop or kneel; reach with hands and arms; and walk distances through rough and uneven terrain.
