Job Overview
We are seeking a dedicated and compassionate Family Support Worker to join our team. The Family Support Specialist conducts home visits for minority pregnant women and families with children under the age of 9, assisting them in accessing essential resources. This role involves identifying eligible families, connecting them with services, collaborating with community agencies, and partnering and recruiting members for housing assistance programs. The ideal candidate will possess strong interpersonal skills and a commitment to empowering families through effective communication and resource management.
Key Responsibilities
Home Visits & Family Support
- Conduct assessments to identify the needs of families and develop tailored support plans.
- Negotiate solutions between families and service providers to ensure access to necessary resources.
- Conduct regular home visits to assess the needs of pregnant women and families with young children.
- Provide information, referrals, and support for healthcare, childcare, nutrition, and housing.
- Offer guidance on parenting, early childhood development, and maternal health.
- Offer crisis intervention services, assisting families during critical situations with effective crisis management strategies.
- Collaborate with community organizations and agencies to connect families with additional support services.
Recruitment & Program Enrollment
- Provide guidance on property management issues, including navigating Fair Housing regulations and Section 8 processes.
- Recruit individual/families into housing assistance and other relevant programs.
- Assist families with application processes for housing, healthcare, childcare, and financial assistance.
- Facilitate access to addiction counseling resources for families dealing with substance use issues.
Data Collection & Reporting
- Track and monitor individual/ family progress to ensure successful program participation.
- Utilize software platforms for managing family support cases and documentation.
- Maintain accurate records of family interactions, progress, and outcomes in compliance with legal standards.
- Maintain accurate records of visits, referrals, and family progress.
- Submit reports on outreach efforts and program effectiveness.
Qualifications
- Education: Bachelor's degree in social work, public health, human services, or a related field (or equivalent experience). A degree in social work, psychology, or a related field is preferred but not mandatory; relevant experience will be considered.
- Experience: Previous experience in home visiting, case management, community outreach, or social services preferred.
- Knowledge of Fair Housing regulations and Section 8 housing programs is highly desirable.
- Background in addiction counseling or crisis intervention is a plus.
- Strong negotiation skills to advocate effectively for families' needs.
- Ability to manage crises calmly and effectively, ensuring the safety and well-being of all parties involved.
- Excellent communication skills, both verbal and written, with a focus on building rapport with diverse populations.
- Skills: Strong communication, cultural competency, problem-solving, and relationship-building skills.
- Requirements: Reliable transportation and willingness to travel for home visits.
