The Director of Social Services is responsible for functioning as the social worker for all levels of living at Summit Square. The primary focus will be on the 18-bed Skilled Nursing Center and the 59-bed Assisted Living area (20 Assisted Living, 22 Catered Living, and 17 Memory Support), ensuring the needs of all residents are met while complying with Virginia Department of Health regulations for long-term care and the Virginia Department of Social Services for Assisted Living.
The Director will work with residents in the community by identifying their psychosocial, mental, and emotional needs along with providing, developing, and/or aiding in accessing services to meet those needs. They are responsible for fostering a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity. This climate shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Requirements
Required:
· Bachelor’s degree in social work or related field from an accredited college or university.
· Valid Virginia driver’s license and meets driver eligibility requirements per Sunnyside Communities policy.
· Customer service skills; maintain a hospitality approach when engaging all individuals.
· Strong verbal/written communication and computer skills.
Preferred:
· Master’s degree in social work or equivalent experience.
· Experience working with the elderly population.
· Minimum of 3 years working in a long-term care setting.
· Knowledge of state and federal Medicare/Medicaid and Nursing Home and Assisting Living regulations.
· Experience with MDS.
· Prior experience with company-utilized software – PointClickCare (PCC), Paylocity, OnShift, etc.
Essential Job Duties:
· Participate as an active member of the Summit Square Leadership Team. Assist with providing strategic direction for the community.
· Work closely with the Director of Health Services and clinical team to ensure all medical/social related issues with residents and families are handled appropriately.
· Coordinates all move-ins and move-outs to/from Health Care (long-term and skilled), Assisted Living, Catered Living, and Memory Support. Ensure the moves are a smooth transition for residents, families, and team members.
· Notify all appropriate team members/families of move-in and move-outs. Make calls/visits as needed to residents in hospital/other communities and update the Resident Review Team as necessary. Submit information to Status Change Hospital email group as needed.
· Contact resident families on an individual basis to share and/or gather information and to gain/provide support. Keep face sheets current with the latest family contact information.
· Ensure occupancy in Health Care, Assisted Living, Catered Living, and Memory Support is maintained at projected budgeted levels completing all paperwork timely and per regulatory requirements/Summit Square policies.
· Actively participate in MDS preparation and care/service planning process by coordinating all care/service plan conferences and completing required documentation. Document all communication/visits with residents/families in medical records.
· Complete and maintain, per federal and state regulations, social history and psychosocial assessments for each resident that identifies social, emotional, and psychological needs.
· Function as the Grievance/Abuse Coordinator for Summit Square. Ensure residents are free from abuse, neglect, financial exploitation, and loss of property by investigating all allegations. Complete all paperwork required by regulatory agencies and Summit Square policies - submit paperwork to the Executive Director.
· Provide guidance, counseling, and direction to residents and/or POA’s regarding financial questions and decisions while staying abreast of current Medicaid requirements, fellowship assistance requirements, and any other financial programs.
· Identify and meet the special needs requirements of residents and use all relevant resources to provide necessary services as the needs arise.
· Analyze and comprehend policy manuals, handbooks, and various state/federal laws/regulations to ensure Summit Square’s compliance. Can interpret policies to all audiences.
· Responsible for compliance with the Resident Council Meetings, including but not limited to resident rights and all hazards review. Approve Resident Council Meeting minutes providing appropriate guidance.
· Attends weekly Resident Review meetings, and other meetings as assigned, representing psycho/social needs of the residents.
· Annually revises and distributes handbooks to health care, assisted living, catered living, and memory support residents. Ensures all new residents receive the appropriate handbook upon move-in.
· Adhere to all company policies and procedures.
· Attend all in-services, training, and meetings as required by company policies and Federal/State Regulations.
· Demonstrate a commitment to safety by following safety rules and guidelines; follow good housekeeping practices, take proper care of equipment, and perform job duties and responsibilities in a safe and responsible manner. Uses chemicals safely and appropriately.
· Demonstrate dependability, reliability, and punctuality. Report to work when scheduled, ready to work.

This job operates in a general office environment with normal lighting levels, temperature ranges, air quality, and ventilation and noise levels.
Work Schedule:
Work assignments and schedules are based on resident needs and staffing requirements and may be changed as needed.
