Job Description:
Summary:
Under indirect supervision, provides, oversees, and/or administers a wide range of psycho-social evaluation/assessment, diagnostic, counseling therapy, crisis intervention services in a high-intensity clinical environment which requires a high degree of independent decision-making. Assigns appropriate staff to clients as clinically indicated. Provides clinical expertise, guidance, and supervision to CST team members .May be asked to lead, or train lower level counselors, students and/or interns engaged in therapeutic patient care activities, as appropriate to the individual position.
Essential Duties and Responsibilities:
The responsibilities of the Community Support duties include, but are not limited to, the following:
Consumer Education / Advocacy

  • Ensures that consumer/legally responsible person receives information regarding rights and assistance as needed in exercising rights
  • Assists consumer/legally responsible person in obtaining information about available service providers (e.g. assists in scheduling of interviews as requested, assists in making contact with providers)
  • Arranging/Planning/Documenting Services
  • Ensures that person centered information is gathered and that the consumer’s health and safety risks are assessed prior to the development of the Person Centered Plan.
  • Facilitates person centered planning and development of the Person Centered Plan (PCP), including Behavior Support Plan as indicated. A copy of all treatment goals must be included in or attached to the PCP.
  • Ensures that the treatment team discusses the consumer’s clinical need for services and that information to support medical necessity is included in the PCP to obtain initial service authorization.
  • Facilitates provider choice process, maintaining objectivity and providing fact-finding assistance.
  • Ensures that services and supports are arranged based on when the consumer needs and wants those services and supports. Services must be arranged to meet the consumer’s needs, rather than for the convenience of staff.
  • Sends a copy of the PCP to all members of the treatment team (e.g. providers, consumer/legal guardian) before services begin.
  • Ensures that signed Authorization to Disclose Health Information forms are obtained and on file in the consumer’s medical record prior to releasing any information.
  • Ensures that all information released/disclosed is documented on the Accounting of Release and Disclosure form (this includes documenting any documents given to consumer/legal guardian).
  • Recognizes and reports critical incidents.
  • Assists consumer/legal guardian with grievances when necessary.Monitoring
  • Monitors the consumer’s services, the consumer’s response to the service and determines with the treatment team the need for adjustments in the service.
  • Documents monitoring and the actions taken/planned as a result of the monitoring in the consumer’s record.
  • Ensures that the monitoring schedule for each consumer is sufficient to assure the health, safety and welfare of the consumer.
  • Monitors for progress/lack of progress through observation, interview, and documentation review. The provider agency is required to make service documentation accessible to the Community Support Staff for review. The Community Support Staff should pay particular attention to the target dates for the consumer’s goals and coordinate efforts with team to ensure that billing in not continued beyond the projected target date. Natural Support / Services Not Funded Through the LME
  • Assists consumer/legally responsible person in considering and accessing natural community supports such as educational services, transportation, support from friends/family/church, etc.
  • Maintains awareness of all services/supports the consumer is receiving and how these services/supports are being provided.
  • Collaborates with others involved with the consumer to help ensure proper care and treatment, prevent duplication of services, and coordinate the services with the LME funded services.
  • Assists the consumer/legally responsible person as needed in working with other service providers (e.g. school system, DJJ, DSS).
  • Ensures that the consumer gets the best possible treatment and care by carefully coordinating paid supports/services with other resources available in the community. Paid Supports (funded by Medicaid, State, and Innovations) are not intended to replace or duplicate services and resources that are available to the consumer in the community. Coordination
  • Works closely with the consumer/legally responsible person, provider agencies, and others involved with the consumer’s care and treatment to avoid/resolve scheduling conflicts, duplication of effort, and other problems that hinder effective treatment.
  • Encourages sharing of important information among all team members, both verbally and in writing. All involved parties need information to provide quality care. Examples of team members include providers, DJJ, DSS, school system, etc.
  • Provides the clinical supervision of all members of the team for the provision of this serviceChanges in Care
  • Ensures that the PCP is updated as needed. When monitoring reveals a change in the consumer’s needs, situation or condition, the Community Support Staff, along with the consumer/legally responsible person and treatment team, should consider updating the Person Centered Plan. Possible changes must be discussed with the consumer/legally responsible person and treatment team. Coordination – Medicaid / NC Health Choice Eligibility
  • Assists consumer in obtaining Medicaid and/or NC Health Choice whenever possible.
  • Monitors the consumer’s continued eligibility for Medicaid and/or NC Health Choice, as applicable, and provides needed assistance to the consumer/legally responsible person in order to ensure that coverage does not lapse.
  • Provides coordination, along with the consumer/legally responsible person, for consumers who have Medicaid deductibles. Special Attributes:

Strong psycho-social, clinical assessment skills. Minimal supervision regarding use of time, able to prioritize work assignments. Ability to communicate effectively with professions and clients/families. Ability to make sound decisions in emergency situations.
Supervisory Responsibilities:
Provides the clinical supervision of all members of the team for the provision of this service
Qualification Requirements:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Education and/or Experience:
Doctorate or Master’s degree in Psychology, Social Work, Counseling or Marriage and Family Therapy, from an accredited college or university and one or more year of experience working as a paid professional clinician. Supervisory experience preferred.
Certificates, License, Registration:
Provisional or full licensed with NC Social Board; Licensed or Certified in Area of Practice: Licensed Clinical Social Worker, Licensed Psychological Associate, Licensed Professional Counselor, Licensed Clinical Addictions Specialist, or Licensed Marriage and Family Therapist.

If provisionally licensed, must be fully licensed within 30 months from date of employment.

Preferred Skills:
Language Skills:
Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, effective treatment notes and related documents. Ability to effectively present information and respond to questions from groups of managers, clients, customers and/or the public.
