Purpose
To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.
Qualifications
- Must successfully complete the required pre-employment Background and Drug Screenings
- Must possess a valid Driver's License which meets company standards for approved driver status
- Must possess advanced computer skills
- Must possess a High School Diploma or equivalent, Bachelor’s Degree preferred
- Experience working in the field of intellectual/developmental disabilities preferred
- Minimum one year of experience in writing program plans, case notes or other relevant documentation required
Essential Functions
- Responsible for the development, implementation and coordination of individual’s program plans
- Develop, implement and monitor individualized training programs to meet identified priority training needs
- Ensures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible
- Ensure all documentation is accurate and complete according to regulatory requirements
- Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
- Performs other duties as assigned
