The medical-psychiatry inpatient program at Oakbend Medical Center in Richmond, TX area is seeking an experienced LMSW to assist patients to foster their capacities and coping mechanisms in order to improve and better their daily lives and experiences. Our patients, experiencing mental, emotional and behavioral challenges will depend on your compassionate care and medical expertise during their healing process. Experience with geriatrics preferred.
Responsibilities
- Plan, coordinate, manage and implement direct social services to patients in accordance with established methods and procedures of the unit.
- Perform psychosocial assessments of patients' current conditions, needs, strengths and weaknesses
- Address each case as a unit and set tailored measurable goals
- Monitor and evaluate patients’ progress and modify treatment plans accordingly
- Conducting and supervising individual, group and family therapies; including daily patient group therapy
- Maintain accurate records and report on patients’ status
- Serve as a liaison between physicians and families
- Act as a key-worker and cooperate with multidisciplinary teams
- Provide discharge planning and follow-up including resource development and assistance
- Supervise PRN social workers and the recreational therapy program
Skills
- Proven work experience as a social worker
- Working knowledge of social theories and practices
- Social perceptiveness and empathy
- Ability to build and maintain professional helping relationships
- Ability to relate and communicate with diverse population and groups
- Resilience along with ability to assess situations
- Good organizational skills and the ability to meet deadlines are required
- Masters degree in social work
- LMSW required
