We offer pathways to housing for people experiencing homelessness through low-barrier access to wraparound services by providing innovative, trauma-informed, and person-centered programming that engages our communities and leads system-wide transformation. Through the Sunrise Hub day center, Sunrise Hotline call center, Sunrise Mobile outreach and shelter services, and Sunrise Housing program we serve almost 20,000 people experiencing homelessness each year.

Job Title: Mobile Wellness Case Manager

Position Status: PT Hourly

Location: In-person

Compensation: $25

Reports To: Wellness Program Manager

FLSA Classification: Non-exempt

Position Summary:

The Mobile Wellness Case Manager supports the Mobile Outreach Team in providing high-quality, integrated care coordination to clients experiencing homelessness in encampments and other locations around Travis County. This role focuses on triaging, assessing, and navigating individuals to appropriate services, including housing assessments, benefits applications, and treatment goal development.

As an outreach coordinator and client advocate, the Mobile Wellness Case Manager emphasizes collaboration and continuity of care by connecting clients with Sunrise Hub and partner agencies and resources in the Continuum of Care (CoC). This medium-touch case management approach ensures clients receive necessary services without maintaining an intensive caseload. The position also serves as a liaison between Sunrise teams and external partners to support effective service delivery for clients with varying levels of need.

This position is funded through a 9-month grant until 09/30/2025, with the intention of securing alternative funding sources to sustain the role beyond the initial term.

Position Qualifications:

  • Bachelor’s degree required; Master’s degree (MSW, MPH, LPC, CHW) preferred.
  • Experience with trauma-informed, harm-reduction approaches in homelessness, mental health, substance use, and medical care.
  • Strong organizational, interpersonal, and communication skills.
  • Proficiency in managing client data and collaborating with multiple agencies.
  • Valid Texas Driver’s License with an acceptable driving record.
  • Minority applicants and individuals with lived experience of homelessness are encouraged to apply.

Position Responsibilities

Client Assessment and Care Coordination

  • Conduct housing and coordinated assessments with clients in the field.
  • Assist clients in developing personalized treatment goals and connect them to partner agencies such as CUC, MAP, and other service providers
  • Assist with benefits signups for Social Security and state benefits
  • Act as the primary liaison between Sunrise teams, external partners and health systems to coordinate care and services.
  • Maintain accurate, clear documentation of client progress and engagement using HMIS, Therapy Notes, and other data systems.

Medication Management

  • Coordinate medication pickup/delivery or connection to Hub Wellness Case Manager for the ordering, secure storage, and distribution of medications, ensuring compliance with safety protocols and client access to essential prescriptions.

Continuity of Care

  • Foster collaboration between Sunrise resources and partner organizations to ensure comprehensive wellness services.
  • Facilitate communication between clients and service providers, minimizing barriers and enhancing service delivery.

Program Support and Data Management

  • Monitor program metrics and ensure compliance with organizational and funding requirements.
  • Contribute to reporting processes and assist in tracking program outcomes.

Client Education and Empowerment

  • Educate clients about available wellness services, promoting self-sufficiency and informed decision-making.
  • Connect clients to medical care, mental health services, and substance use programs.

Community Outreach and Collaboration

  • Engage with partner agencies to facilitate outreach and provide seamless service delivery.
  • Attend community meetings and build relationships with key stakeholders to enhance service capacity.

Position Considerations:

  • Schedule: Part-time schedule totaling 30-32 hours per week, with weekday shifts typically from 9 a.m. to 3 p.m., subject to variability.
  • Collaboration: Work closely with internal teams and external partners.
  • Work Environment: Combination of fieldwork, office work, and shelter settings.
  • Physical Requirements: Tasks may require bending, lifting up to 35 lbs., and travel between sites.
  • Outdoor Exposure: Potential exposure to outdoor elements such as heat, cold, dust, and noise.
  • Transportation: Use of personal transportation for client engagement and meetings; mileage reimbursed.
  • Adaptability: Responsibilities may evolve as Sunrise's operations expand.
